This application will be <br />evaluated utilizing the criteria outlined prtviorttly. <br />A. Aoolication <br />The application shall require, at a minimum: <br />1. Name, and addrus of the applicant; <br />2. Site address and location; <br />3. Fariliry romaa Anson and telephone number,• <br />4. Standard /ndustrial Classification (SIC! rode(sJ; <br />5. The size of the mining operation in acres; <br />6. Status as active ar inaaiHe; <br />7. Lut of any othn environmental pmnits currently held by the facility; <br />8. Aaiviriu whidt take place m the site; <br />9. Receiving warns; and <br />10. For new arrive mining opnmions (beginning opnmions aJln 10/1/92! or existing jacilitiu applying after <br />7/1/93, certification as to the preparation of a SR'MP. (See Section V/I.B ojthe Rationale far SWMP <br />cert~cation rcquirnnems for existing mining opnaiioas applying an time!. Inactive mining opnmions <br />applying aftn July 1, 1994 must submit a eerthcation as to the prcparmion and impkmemmion of a SWMP. <br />B. Procersin¢ Trme <br />If this gcneral permit rs suitable for the applirom's opnmion, then a certification will be developed and the <br />applicam will be coshed under this general permit. <br />The Division shall haHe up to thirty days aJtn receipt of the abort information to requut additional data and/or <br />deny the authorization for any particular discharge. Upon receipt of additional information, the Division shall <br />have an additional fifteen days to issue or deny aahortzation jot the particular dscharge. <br />If the applicam dues rot receive a requut for additional information or a notification of denial from the Division <br />dated within thirty days of the receipt of the applicmion by the Division, authorization to discharge in accordance <br />with the conditions of this permit shall be deemed grained. <br />If, after evaluation of the application (or additional information, suds ar the SNMP), it is found that this genna! <br />permit is not suitable for the opnmion, then the applicuion may be protrssed as one for an individual permit, or <br />a more appropriate genna/ permit. The applicant will 6e noted of the Division's decision to deny certifcmion <br />under this general permit. For an individual permit, additional information may be requested, and ]80 days will <br />6e required to process the application and issue the permit.
