Compliance Report for Laactive 11Tining Operations: The permittea will be roquirad to submit a report on the <br />compliance with the SWMP. The report will contain, at a minimum: <br />1) A report on the facility's overall compliance with the SWMP. <br />2) Name of permittx, address, phone number, and permit cutification number. <br />3) A summary or copies of the annual inspection reports, or the triennial certification by a Professional <br />Engineer that the facility is in compliance with the permit <br />The report will be due to the Division an or before February 15, 1996 and cover the period of time between the <br />coverage by this general permit end December 31, 1995. The Division reserves the right to require additional <br />information in the report, and moro froquent reports, on a caso-by~ase basis, ee needed. <br />All reports requirod for submittal shall be signed and certified for accuracy by the permittee. (See Part I.D.9 of <br />the permit.) <br />8. Monitorirta <br />The Division reserves the right to require sampling and testing, on a caso-bycase basis, in the event that there is reason <br />to suspect that compliance with the SWMP is a problem, or to measure the effectiveness of the BMPs in removing <br />pollutants in the effluent. <br />Sienatorv Rmuiranents <br />a. All reports required for submittal shall be signed and certified for accuracy by the permittea in accordmce with <br />• the following criteria: <br />1) In the case of corporations, by a principal executive officer of at least the level of vice-president or his or <br />her duly authorized representative, if such representative is responsible for the overall operation of the <br />facility from which the discharge described in the form originates; <br />2) In the case of a partnership, by a general partner; <br />3) In the case of a sole proprietorship, by the proprietor, <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking <br />elected official, or other duly authorized employee. <br />b. Changes to authorization. If an authorization under paragraph a. of this section is no longer accurate because <br />a different individual or position has responsibility for the overall operation of the facility, a new authorization <br />satisfying the requirements of paragmph a. of this section mast be submitted to the Division, prior to or together <br />with any reports, information, or applications to be signed by an authorized representative. <br />a Certification. Any person signing a document under paragraph a. of this section shall make the following <br />certification: <br />'I certify under penalty of law that this d ant and all attachments were prepared under my direction <br />or supervision in accordance with a system designed to assure that qualified personnel property gather and <br />evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, <br />or those persons directly responsible for gathering, the information submitted is, to the best of my <br />knowledge and belief, true, accurate, end complete. I am aware that there are significmt penalties for <br />• submitting false information, including the possibility of fine and imprisonment for (mowing violations. <br />
