Stunmary of Application: The application requires, u a minimum, the following: • 1) Name, and address of the applicant; 2) Site address and laration; 3) Facility contact person sad telephone number, 4) Standard Industrial Classification (SIC) code(s); 5) The sve of the mining operation in acres; 6) Status as active or inactive; 7J List of say other environmental permits currently held by the facility; 8) Activities which tape place at the site; 9) Receiving waters: and 10) For new at~ve mining operations (beginning operations after 10/1/92) or existing facilities applying after 7/1/93, ceRificative as to the prepsrative of a SWMP. For inactive mining operations applying after July 1, 1994, catifitstion as W the preparation and implernmtatioa of a SWMP. c. Certification of SWMP Completion and Lnpl®rentation For active mining operatirras, existitg ve October 1, 1992, the permittee shall have implemented and be in compliance with their SWMP, as coon as possible but ao later than the aveiversary date of their Mined Land Rechunative permit thu falls between July 1, 1993 and lone 30, 1994. Certification to this effect must be received by the Division on or before that date. For sew active fadlitia (beginning opcruives after October 1, 1992), and for all other active facilities applying after July 1, 1993, the SWMP shall be prepared prior to appiyiag for coverage tinder the general permit. For inactive mining operations, applying before July 1, 1994, as soon as possible but no later than July 1, 1994, the permittce shall have implemented and be in cotttpliance with their SWMP. Certification to this effect must be received by the Divisive ve or before that date. For those applying after July 1, 1994, the SWMP shall be Prepared Prior to applying for coverage tender the general permit. 4. Permit Certification Prnoedtass If this general permit is appropriate for the applicant's operative, they a certification will be developed and the applicant will be authotired to discharge stormwuer order this general permit. a Request for Additional Information: The Division shall have up to thirty days after receipt of the above information W mgttest additional dun and/or deny the authorimtive for any particular discharge. Upon receipt of additional information, the Division shall have as additional fdteen days m issue or deny authorvation for the particular discharge. Temporary coverage order this general permit will be allowed until the individual permit goes into effxt b. Automatic Covtiage: If the applicant does not receive a toques[ for additional informative or a notification of denial from the Divisive dated within thirty days of receipt of the applicative by the Division, authoriration to discharge in accordance with the conditions of this permit shall be dcemed granted.
