The final design of all future warranted highway improvements shall be submitted [o [he Department for <br />review and approval prior to commencement of any construction within the highway right-o(-way. The <br />design shall be provided by x Colorado register ed professional engineer. <br />16. Any future warranted highway improvements shall be designed and constructed by the Permi[tee or the <br />property owner's at no cost to the Department. Anew access application shall be submitted and a new access <br />permit shall be issued by the Department prior to an increase in the herein-permitted vehicular volumes(s) or <br />any additional highway constrvction activities. Any warranted highway improvements shall be installed <br />within the earliest construction season. Under no cucumstances will [he access be allowed to operate in an <br />unsafe manner. <br />17. The design and construction of any future warranted highway improvements shall comply with the following <br />current editions: <br />a. Roadway Design Manual <br />b. Materials Manual <br />c. Construction Manual <br />d. Standard Specifications for Road and Bridge Conswction <br />e. Colorado Standazd Plans <br />f. Manual on Uniform Traffic Control Devices for Streets and Highways and the Colorado <br />Supplement thereto. <br />g. A Policy on Geometric Design of Highways and Streets, 1999 edition. <br />h. Transportation and Traffic Eno veering Handbook. <br />i. Trip Generation Manual <br />j. State Highway Access Code Volume 2, Code of Colorado Regulations 601-1 <br />k. Roadside Design Guide <br />18. This permit is issued in accordance with the 1993 State Highway Access Code and is based upon [he <br />information submitted by the Permittee. Any chanties in the herein permitted type and use and/or volume of <br />traffic using the access, drainage, or other operational aspecu may render this permit void, requiring that a <br />new application be submitted for review based upon currently existing and anticipated future conditions. <br />Installation of auxiliary lane improvements may be required if a safety problem is created on the highway <br />system or if future vehicular volume warrants are met. as required by the State Highway Access Code Volume <br />2 Colorado Code of Regulations 601-1 or the Manua] on Uniform Traffic Control Devices. <br />19. You must contact Ralph Martinez at 401 Monroe in Monte Vista or call {719) 852-5737 to meet with him at <br />feast two (2) working days prior to beginning anv access improvements or construction of any kind within the <br />highway right-of-way. This includes but is not limited to the installation of side drain eulverLS and fencing. <br />Failure to comply with this condition may result in revocation of this permit. <br />20. A construction traffic control plan has been enclosed as pan of this permit (EXHIBIT C). If permittee has any <br />traffic control questions please contact the Region 5 Traffic Engineer (3805 North Main Avenue, Suite 100, <br />Durango, Colorado 81301, or telephone (970) 385-8360. All traffic control shall be in conformance with the <br />Manual on Uniform Traffic Control Devices, and all other applicable standazds. No lane closures or traffic <br />detours of any nature will be allowed unless pre-approved by the Department <br />21. A FULLY EXECUTED COMPLETE COPY OF THIS PERMIT AND THE VALIDATED NOTICE TO <br />PROCEED IvfUST BE ON THE JOB SITE WITFI THE CONTRACTOR AT ALL TL~IES DURLNG ANY <br />CONSTRUCTION OF THIS ACCESS. FAII-URE TO COMPLY WITH THIS OR .4i~' OTHER <br />CONSTRUCTION REQUfREIvfEIdT' MAY RESULT IN THE ibLtitEDIATE SUSPENSION OF WORK <br />BY ORDER OF THE DEPARTMENT OF TRANSPORTATION. <br />22. The Permittee(s)1Propeny owner(s) are hereby notified that the location, configuration, or design of this <br />access may be altered by CDOT during future highway construction. Leh turn movements in and out of this <br />access may be prohibited at some future date. <br />
