Permit Revisions <br /> PR-02 proposed approval was issued December 5, 1997. The revision was submitted, in part,to address <br /> deficiencies and inconsistencies identified in the Division's August 18, 1995 mid-term permit review, <br /> and in part to organize the permit application package into a more useable format. The previous <br /> permit application package was completely replaced by a new six volume permit application submitted <br /> as PR-02. The format of the documents was completely reorganized, and significant and substantive <br /> changes were incorporated in various sections of the application. The revision did not entail expansion <br /> of disturbed, affected,or permitted acreage, beyond that previously set forth in the approved permit. <br /> The Division's December 10, 1997, "Proposed Decision and Findings of Compliance for a Permit <br /> Revision, Roadside North and South Portal Mines",was prepared pursuant to PR-02. <br /> Technical Revisions <br /> TR-16 proposed approval was issued January 4, 1994. The revision allowed for a reduced cover depth on <br /> approved coal processing waste disposal areas, and addressed details regarding reclamation sequence, soil <br /> handling, and vegetation monitoring on final reclaimed areas. <br /> TR-17 proposed approval was issued April 19, 1994. The revision established two borrow areas for refuse <br /> cover material (CBA#1 and CBA #2), and allowed for the initial phase construction of Sediment Pond #1 1. <br /> Prior to activation of CRDA No. 3,enlargement of Pond I I would have been required. <br /> TR-18 proposed approval was issued October 13, 1994. The revision allowed for reactivation ofCRDA No. <br /> I, which had been suspended due to potential stability concerns. Reactivation was approved based on <br /> completion of a thorough stability analysis,and commitment to intensively monitor water levels within the pile. <br /> TR-19 proposed approval was issued January 30, 1995. The revision allowed for excavation of a connecting <br /> channel between Sediment Ponds I and 2, and replacement of Pond 7 and 8 CMP spillways with riprapped <br /> open channel emergency spillways. <br /> TR-20 proposed approval was issued December 28, 1995. The revision addressed increase in coal production <br /> and underground mining sequence changes associated with reactivation of mining in the Roadside North <br /> Portals. Modifications to hydrology and subsidence monitoring requirements within the North Portals affected <br /> area were included in the revision. <br /> TR-21 proposed approval decision was issued with one stipulation on March 6, 1995. The revision allowed <br /> for expansion and modification of the coal handling area at the Roadside North Portal. Stipulation No. 14, <br /> attached to the approval, required submittal of a required air emission permit prior to implementation. An <br /> "Initial Approval" construction permit from the Air Pollution Control Division dated June 27, 1995, was <br /> subsequently submitted to the Division, as required by the stipulation. <br /> TR-22 proposed approval decision was issued October 16, 1997. The revision allowed for construction of a <br /> minewater treatment pond (Pond 12),adjacent to existing Pond 5,at the North Portal facility area. The pond <br /> was proposed to provide for surface treatment of minewater from the North Portal,discharged through CDPS <br /> Point 004. Suspended solids discharge limits had been exceeded at the location on several occasions. The <br /> revision included associated modifications to adjacent Pond 5, to accomodate Pond 12 construction. <br /> 7 <br />
