<br />4 <br />"Fold Away" portable shop over a concrete floor. The operator demonstrated that shop <br />and concrete floor demolition costs would be lower than previously estimated. The <br />Division issued a proposed approval decision on February 4, 1998, which set forth a <br />revised liability amount of $161,499.00. This Lability amount exceeded the bond amount <br />held by $352.00. The revision was not implemented and the additional bond amount has <br />not been submitted. <br />MR 26 was submitted to the Division on February 11, 1998, and deemed complete on <br />February 23, 1998. The revision removed reference to John Mcnab as president of Grand <br />Valley Coal Company, and provided an updated list of mines with which GVCC owners <br />and controllers aze affiliated. This revision was never approved. In a letter of August 25, <br />1998, the Division addressed one AVS (Applicant Violator System) related item which <br />needed to be resolved prior to approval of the revision, and two AVS related items which <br />need to be resolved prior to any future permit renewal, succession of operator, or permit <br />revision. <br />TR 09 was submitted to the Division on October 17, 1997 and deemed complete on <br />October 27, 1997. The revision submitted information in response to Stipulations 23 <br />through 29, which had been attached to the Division's Permit Renewal No. 4 proposed <br />decision, as well as to the subsequently issued renewal of Permit C-80-004. With the <br />operator's most recent adequacy response submittal of November 24, 1998, all issues of <br />concern pertinent to the stipulations had been satisfactorily addressed. Associated with <br />review of TR-09, the Division performed a reclamation cost estimate which included the <br />additional tasks associated with the revision, and in addition updated the costs as required <br />for this mid-term review. Approval of TR-09 is pending review of the estimated <br />reclamation cost amount by the operator. <br />
