<br />June 3, 1994 <br />2 <br />943-2720.010 <br />If shear zones exhibiting resurgence are encountered during the foundation preparation, <br />verify that the subdrain has been appropriately configured and sized to receive and <br />transport the groundwater flow. The operator should also take additional precautions to <br />assure the integrity of the artificial flow barrier installed above the underdrain. <br />Immediate Notice of Anomalous GenloEical & Hydro~eoloeical Conditionc• <br />Inform the OMLR immediately of any foundational conditions encountered which deviate <br />from those projected within the permit application. Mapping and documentation shall <br />be kept up to date and provided to the Division weekly. <br />Inspection Records: <br />Maintain daily inspection logs and field test and laboratory analytical results and geology <br />and hydrogeology maps on the site. Provide to OMLR personnel for examination upon <br />request. Summary reports prepared by Golder to be submitted to the Division within a <br />week of their completion, as well as maintained at the site. Final geology and <br />hydrogeology maps shall be provided to the Division as a portion of the certification <br />reports. <br />Certification for Valley Leach Facility <br />Certification reports, prepared and signed by Golder, shall be submitted to the Division <br />by CC&VG for the entire heap leach facility. Separate certification reports may be <br />submitted for portions of the heap leach. These reports shall include all weekly <br />inspection summaries and footprint geology and hydrogeology maps prepared by Golder. <br />The certification reports shall address all design and construction components of the <br />facility, commencing with the foundation footprint preparation and terminating with the <br />installation of the protective liner cover material above the uppermost composite liner. <br />All testing conducted in determining compliance with specifications for construction and <br />performance of the components of the facility shall be included in appendices to the <br />certification reports. No exclusions from certification will be accepted for any reason. <br /> <br />No ore shall be placed within any portion of the heap leach facility until the certification <br />report for that portion of the facility has been accepted in writing by the Division. <br />
