,Y..: ... ,.....,: <br />...... <br />LIST.OF EQUIPMENT OFFERED <br />The list of Equipment Offered form must be returned with each bid. All equipment to be <br />used on the project must be listed. The CONTRAC'POR must own, have a current lease <br />for the equipment intended to be used to perform the work, or must furnish a statement of <br />arrangements to rent, or otherwise procure, adequate equipment to rform the work. <br />Inclusion of a pies of equipment on the List of Equipment Offered represents a <br />commitment to devote that piece of equipment to the project work for the entire duration <br />of the project. If several contractors propgse to join together to perform the work lender <br />this purchase order contract, equipment owned or controlled by each should be listed and <br />the owner identified A purchase order contract will not be awazded to a bidder who in the <br />opinion of the OWNER (a) does not have adequate equipment, or (b) does not have firm <br />and satisfactory arrangements to obtain adequate equipment to perform the work descn'bed <br />in the Invitation and Bid. <br />PROJECP FUNDING <br />This project is funded 100% through monies made available to the State of Colorado <br />through revocation of bond monies retained by the Division of Minerals and Geology for <br />reclamation of this previously permitted mining operation. <br />AMERICANS WITH DISABILITIFS ACT <br />All programs, services and activities of the Department of Natural Resources, Division of <br />Minerals and Geology and the Colorado mined Land Reclamation Board are operated in <br />compliance with the Americans with Disabilities Act. Questions, complaints and requests <br />for additional information may be directed to ADA Coordinator, Department of Natural <br />Resources, 1313 Sherman Street, Room 718, Dem~er, CO 80203. <br />PROJECT WORg DESCRIPTION <br />The following tasks will be performed on this Project: <br />Task #1: Mobilization and Demobilization <br />Task #2: linden Avenue Loadout coal removal and reclamation <br />Task #3: Section IS stream realignment <br />Task #4: Pond 15-P10 removal <br />Task #5: Reconstruction of Pond ?.2-P10 <br />Task #6: Ditch repair, culvert installation, rock swale construction <br />Task #7: Spillway repair and clean out of Pond 15-P20, and removal of associated ditch <br />Task #8: Spillway removal pond 22-P4 <br />Task #9: Ditch and gully repair <br />Task #10: Spillway repair ponds 15-Pll and 15-P]2 <br />Task #11: Riser modification, Ponds 22-P5, 22-P6, and 22-P10 <br />Task #12: Trailer removal <br />Task #13: Revegetation <br />Task #14: Non-guaranteed task -reforestation of azeas near Ponds 15-P20 and 22-P17 <br />Task #15: Non-guaranteed task -Pond 15-P1 clean-0ut <br />
