The following table summarizes the stipulation status for CCMC. <br />Revised pages should be inserted into the permit which indicate <br />commitment to all stipulations which have been listed as satisfied. <br />In addition, sufficient information must be presented to address all <br />outstanding issues for stipulations which have been listed as not <br />satisfied This information should also be submitted as revised <br />pages to the permit. <br /> <br />
