All maps, tables and figures that are applicable to <br />the test pit should be modified as necessary to depict current <br />site conditions. <br />3. Section 2.03 - Financial and Legal Compliance <br />a. The identification of interests, mine permit, right of entry <br />information, and insurance policy status should be updated as <br />necessary. <br />b. The time frames contained in Section 2.03.11-12 should be <br />revised to reflect the current status of the mining operations. <br />c. Page 2.04-18 is missing in all copies of the permit application <br />document. Please insert or explain why this page has been <br />deleted. <br />d. The compliance information presented in Section 2.03.5 should <br />be updated to include all enforcement actions to date. A <br />discussion of each violation, including its nature and <br />resolution, should be included. Division records indicate that <br />the following enforcement actions should be included in this <br />section: <br />82-83 <br />83-28 <br />84-011, 012, 034, 035, 036, 037, 038, <br />065, 069, 125, 126, 127, 128, <br />129, 144, 155, 161 <br />85-017 <br />86-028 <br />4. Section 2.04 Minimum Environmental Resources Information <br />a. Narratives which discuss overburden geochemistry and soil <br />•~ chemistry should be revised to reflect any changes due to the <br />dry mine plan. The deletion of mining in lower strata and <br />associated changes in the reclaimed spoils composition should <br />be considered. <br />b. The geology description included in Section 2.04.6 must be <br />updated to show the added overburden geochemistry parameters <br />required by the permit findings document. These parameters <br />include: <br />Iron Sulfate Chloride <br />Copper Barium Fluoride <br />Nickel Cadmium Mercury <br />Zinc Chromium <br />If available, data for these parameters should be inserted in <br />Tables 4 and 5. <br />
