Response: Complete and accurate maps submitted herewith. Descriptions of all sites and <br />facilities within and associated with this Permit are described herein. <br />One Portal on Map: Map submitted by the Operator in April 1998 showed only one portal. <br />Response: There are two portals within the Sunday Mine yard. The "Old Sunday Portal" <br />was permanently closed by the Operator in November 2002. The map submitted herewith shows <br />the location of the Old Sunday Portal, but this site was regraded after the current map survey was <br />conducted. The site map submitted herewith (1" = 100') will be updated to show the minor <br />changes associated with the portal closure, and a revised map will be submitted with a future <br />Annual Report to the Division. <br />GMG "New" site: The map submitted by the Operator in April 1998 did not show the GMG <br />mine site and its inclusion under the Sunday Mine Permit. <br />Response: Complete maps submitted herewith. <br />Topsoil salvaged dun'ng operations: Although the original permit materials note that no topsoil <br />was salvaged during PreLaw operations, the Division presumes that topsoil has been salvaged as <br />the mine site expanded. The division requests that the Operator show the location of stockpiled <br />topsoil and a calculation of the volume stockpiled. <br />Response: The "footprint" of the Sunday Mine site has not expanded under operations <br />conducted by the current Operator. To the best of the knowledge of this Operator, the only <br />topsoil that was ever salvaged by past operators at this site is shown on the updated map <br />submitted herewith. The estimated volume of topsoil in this pile is reported in the description <br />provided herein. <br />Show sediment and runoff control structures: <br />Response: The new Sunday Mine map (1" = 100') shows sediment and runoff control <br />structures. <br />December 18, 2001 Inspection by Bob Oswald: <br />Operator has failed to submit current and accurate maps: This item was noted by the inspector in <br />past inspections and correspondence. As a result of the Operator's inaction, this matter was <br />classified as a "problem" and a Corrective Action was specified, with a Correction Date of July <br />25, 2002. <br />Resnonse: The Operator has coordinated with the inspector over the past few months to <br />ensure that the necessary and complete information has been collected. The Operator is now <br />submitting the information stipulated in the Corrective Action. The Operator appreciates the <br />inspector's assistance and cooperation in granting the Operator time to complete the necessary <br />work to address the Corrective Action. <br />The Operator is only submitting mining maps at this time. Submittal of proposed final <br />reclamation maps can be accomplished following review and concurrence by the Division and <br />the Operator on the specific features and elements included under this Permit. <br />Operator has failed to post ID sign: The Operator has not responded to past requests of the <br />Division to post a new Permit ID sign and to delineate the Affected Area boundaries. This <br />matter was classified as a "problem" and a Corrective Action has been specified, with a <br />Correction Date of July 25, 2002. <br />9 <br />
