RESPONSES TO INSPECTION REPORTS July 10, 1997 Inspection by Bob Oswald and Harry Posey: Permit ID sign: Inspector noted that Energy Fuels sign was still on access gate. Operator indicated that this would be updated. Response: Anew identification sign was placed at the Sunday Mine site entrance in January 2002 after follow-up inspections by the Division. Affected Area boundary markers: Inspector noted that Affected Area boundary was not marked and that this needs to be addressed by the Operator. Response: This item has not been resolved by the Operator and is noted by the Division in subsequent inspections. A proposal on resolution of the boundary is presented in the material submitted herewith. GMG Mine Site: This site was inspected, and the Division noted that this site is to now be considered as a remote site under the Sunday Mine 112 Permit. Power existed to this site, but there was no fan installed in the portal at the time of the inspection. The Division was not aware of the Operator's utilization of the GMG site before this inspection was conducted. Response: No comment. The status of the GMG site is discussed in the update report submitted herewith. Current Summary and Map: In conjunction with the transfer of this Permit to the current Operator, the Division noted that the entire project mapping needs to be made current to serve as a basis for analysis of bond adequacy. Response: The Operator is submitting complete and current project maps herewith. HaloQeton Infestation: The presence of the weed Halogeton was noted by the inspector. Response: No response. Observed Problems to be Resolved: No specific problems or Corrective Action items were noted during this inspection. Response: No response. June 2. 1998 Letter from Division: Man of defined site boundary: Map submitted by the Operator in April 1998 presents a new site boundary; boundary exceeds 20 acre limitation. Response: New maps are submitted herewith and the Operator proposes an Affected Area boundary that encompasses all site disturbances. Allowable disturbance acreage under the Sunday 112 Permit is a total of 40 acres. Map location reference and access road: Map submitted by Operator in April 1998 did not have point of reference to tie to section corner, and the access road was not shown. Response: New maps submitted herewith. Map incomplete: Map submitted by Operator in April 1998 did not include access roads, vent holes, powerlines, ownership information, areas reclaimed, etc.
