RESPONSES TO INSPECTION REPORTS <br />July 10, 1997 Inspection by Bob Oswald and Harry Posey: <br />Permit ID sign: Inspector noted that Energy Fuels sign was still on access gate. Operator <br />indicated that this would be updated. <br />Response: Anew identification sign was placed at the Sunday Mine site entrance in <br />January 2002 after follow-up inspections by the Division. <br />Affected Area boundary markers: Inspector noted that Affected Area boundary was not marked <br />and that this needs to be addressed by the Operator. <br />Response: This item has not been resolved by the Operator and is noted by the Division <br />in subsequent inspections. A proposal on resolution of the boundary is presented in the material <br />submitted herewith. <br />GMG Mine Site: This site was inspected, and the Division noted that this site is to now be <br />considered as a remote site under the Sunday Mine 112 Permit. Power existed to this site, but <br />there was no fan installed in the portal at the time of the inspection. The Division was not aware <br />of the Operator's utilization of the GMG site before this inspection was conducted. <br />Response: No comment. The status of the GMG site is discussed in the update report <br />submitted herewith. <br />Current Summary and Map: In conjunction with the transfer of this Permit to the current <br />Operator, the Division noted that the entire project mapping needs to be made current to serve as <br />a basis for analysis of bond adequacy. <br />Response: The Operator is submitting complete and current project maps herewith. <br />HaloQeton Infestation: The presence of the weed Halogeton was noted by the inspector. <br />Response: No response. <br />Observed Problems to be Resolved: No specific problems or Corrective Action items were noted <br />during this inspection. <br />Response: No response. <br />June 2. 1998 Letter from Division: <br />Man of defined site boundary: Map submitted by the Operator in April 1998 presents a new site <br />boundary; boundary exceeds 20 acre limitation. <br />Response: New maps are submitted herewith and the Operator proposes an Affected <br />Area boundary that encompasses all site disturbances. Allowable disturbance acreage under the <br />Sunday 112 Permit is a total of 40 acres. <br />Map location reference and access road: Map submitted by Operator in April 1998 did not have <br />point of reference to tie to section corner, and the access road was not shown. <br />Response: New maps submitted herewith. <br />Map incomplete: Map submitted by Operator in April 1998 did not include access roads, vent <br />holes, powerlines, ownership information, areas reclaimed, etc. <br />
