located in Sections about the permit azea is provided under sepazate cover. Please expand <br />this portion of the PAP to comply with these rules. <br />Item 8. Rule 2.04.7(4). No map of well locations, at a minimum scale of 1:24,000 in <br />conformance with Rule 2.10, was found in the PAP. Please add such a map to the PAP. <br />Item 9. Rule 2.04.7(4)(a). All the wells within a mile have not been identified. Please <br />identify, in the PAP, all wells within, and within a mile of, the permit area. <br />The permit is in temporary cessation. TR-6 approved certain changes to existing features <br />of the permit azea. However, due to temporary cessation, these approved features have not <br />materialized. Not only does this situation provide a confusing representation of the permit <br />area, but leaves (after posting of TR-6 changes to the PAP) some existing features <br />without authorization or required supporting information. For example, haul roads, access <br />roads, drainage ditches, and structures are in a state of flux. Some of the following <br />comments aze derived as a result of this incomplete transition from existing to approved <br />conditions. <br />Item 10. Please clarify the PAP to describe existing and approved conditions, with <br />supporting documentation for each. Retention of Drawings 12-1, Revision 10; 12-1A, <br />Revision 0 of 5/15/00, and 12-2, Revision 7; and supporting documentation (haul and <br />access road certifications, ditch designs, etc.), with text that explains the transition over a <br />period of temporary cessation might clarify the difference between the maps and text in <br />the PAP and the conditions presently at the site. See Items 11 through 17. <br />_ Item 11. Rule 2.05.3(3)(c). Appendix 12-5 is referenced on page 5 as a haul road (HGLF- <br />HRl)certification. The current (post TR 6) Exhibit 12-5 is a design for haul road HGLF- <br />HR2. Post TR 6 configuration of HGLF-HRl remains unchanged under Technical <br />Revision (TR) 6. (Note that the configuration of HGLF-HRl on Exhibit 12-1, Revision <br />10, 11/2004, is not the original loop-alignment of HGLF-HRI.) Please clarify the PAP <br />to correlate certifications with the appropriate road. <br />Item 12. Rule 2.05.3(3)(c). Please better define the proposed access road on Drawing <br />12-2 (Revision 9). <br />Item 13. Rule 2.05.3(3)(c). A light use road is described on page 8 of Tab 12. The light <br />use road is not shown on a map. Please show the location of the light use road on the <br />appropriate maps. <br />Item 14. Rule 2.05.3(3)(c)(v).
