Status - Terminated <br />The revised permit application included a subsidence monitoring <br />plan that incorporated quarterly monitoring of the Thompson Ranch <br />structures. This monitoring has been performed in accordance with <br />the plan to date. <br />STIPULATION NO. 11 <br />THE DIVISION DIRECTS ENERGY FUELS COAL,INC. TO SUBMIT A PROPOSED <br />REPORT FORMAT FOR THE REQUIRED SEMI-ANNUAL SUBSIDENCE MONITORING <br />REPORT, WITHIN 180 DAYS FOLLOWING PERMIT APPROVAL. THIS REPORT <br />FORMAT SHALL INCLUDE TABULATED SURVEY MONUMENT AND STRUCTURE <br />LOCATION AND ORIENTATION DATA. SURVEY DATA FOR THE TWO <br />HIGH-RESOLUTION MONUMENT GRIDS SHALL ALSO BE PRESENTED GRAPHICALLY, <br />IN CROSS-SECTION FORM. THE SUBSIDENCE MONITORING MAP SHALL BE <br />AMENDED TO INCLUDE MONUMENT IDENTIFICATION LABELING, IN ORDER TO <br />FACILITATE DATA REPORTING AND ANALYSIS. FINALLY, SEMI-ANNUAL <br />SUBSIDENCE MONITORING REPORT SHALL INCLUDE A SUMMARY COMPARISON OF <br />MONITORING RESULTS AND THE APPLICANT'S PREDICTIONS FOR EACH <br />INVENTORIED STRUCTURE AND THE STREAM CHANNEL OF NEWLIN CREEK. <br />''Status - Terminated <br />The Division approved a semi-annual subsidence monitoring report <br />format in conjunction with the first mid-term, Minor Revision 13, <br />approved December 10, 1987. <br />V$TIPULATION NO. 12 <br />PRIOR TO AFFECTING THE LAND IN THE SOUTHWEST ; SECTION 23, TOWNSHIP <br />20 SOUTH, RANGE 70 WEST, THE OPERATOR SHALL SUBMIT RIGHT-OF-ENTRY <br />INFORMATION FROM THE OWNER. <br />Status - Future <br />EFCI has not mined in this area, however this quarter section is <br />within the angle of draw proposed disturbance area. This <br />stipulation has been further addressed in Technical Revision No.18, <br />received June 15, 1994 and currently under adequacy review by the <br />Division. <br />~ STIPULATION NO. 13 <br />WITHIN 90 DAYS OF APPROVAL OF PERMIT REVISION NO O1, THE OPERATOR <br />SHALL SUBMIT DOCUMENTATION THAT APPLICATION FOR A WELL PERMIT FOR <br />THE "NORTH DEWATERING WELL" HAS BEEN FILED WITH THE OFFICE OF THE <br />STATE ENGINEER. <br />Status - Terminated <br />5 <br />
