004 Midterm Review <br />Rule 2.03.2 Responsibilities <br />The applicant has consistently fulfilled its responsibilities associated with this Permit. <br />Rule 2.03.3 Format and Supplemental Information <br />The Division has approved and accepted the format and supplemental information of the Permit <br />by due process. <br />Rule 2.03.4 Identification of Interests <br />In compliance. Changes in interests were made to the PAP upon transfer (Mazch 22, 2002) of the <br />Permit to C.B. Minerals Company, LLC. <br />Rule 2.03.4(1) Organization <br />The applicant is a Limited Liability Company (PAP, page 2-2). <br />Rule 2.03.4(2) Applicant Identification <br />The applicant is C. B. Minerals Company, LLC (Rule 2.03.4(2)(a). The Federal Employer ID No. <br />of the applicant is EIN - 84-1288888. The resident agent is James E. Stover (Rule 2.03.4(2)(b). <br />The payer (person) of the abandoned mined land reclamation fee is the LLC (PAP page 2-2). <br />Rule 2.03.4(3) Interests Identification <br />Officers and Directors of the LLC aze provided on page 2-3 of the PAP. No interest currently <br />listed has other coal mining permits (PAP, page 2-3). The applicant or principal shazeholder <br />previously operated a surface coal mine operation in the U.S. within five yeazs preceding as <br />NCIG Financial, Ino. (PAP, page 2-3). Surface owners and owners of coal to be mined aze <br />provided on page 2-4 of the PAP. There are no lease holders or purchasers of record under a real <br />estate contract. Contiguous ownership is provided on pages 2-5 through 2-7 of the PAP <br />Rule 2.03.5 Compliance Information <br />In compliance (PAP, page 2-7). No mining operation has been suspended or revoked, and no <br />mining bond or similaz document has been forfeited. No violations have been invoked during the <br />last three years, <br />Rule 2.03.6 Right of Entry and Operation Information <br />In compliance. The Coal Ridge No. 1 Mine is reached by public access (Garfield County Road <br />336 from the I-70 New Castle Exit 105). Access onto the site is by an Access and Surface Use <br />Agreement with NCIG Financial, Inc. The permit azea is located in Sections 1 and 12, T 6 S, R <br />91 W, and Sections 5, 6, 7, 8, 9, 15 and 16, T 6 S, R 90 W, 6`" Principal Meridian, and includes <br />2,484.3 acres more or less, of which 45.2 have been disturbed.
