Violations <br />No notices of violation were issued since the fast permit renewal. <br />Revisions to Permit <br />The only revisions made to the permit since the last permit renewal were Minor Revision <br />MR-13 for slump mitigation work and MR-14 for installation of a culvert. <br />Permit Stipulations <br />No stipulations have been added to the permit since the last permit renewal, and all <br />stipulations on file have been complied with. <br />Bond <br />The permittee still has on deposit with the Division a corporate surety bond in the original <br />reclamation liability amount of $2,300,000. By letter of December 19, 1997, the Division 1 <br />authorized the bond issuer, Aetna Casualty and Surety Company, to reduce remaining <br />liability coverage to $551,280. <br />The remaining bond liability is $22,562, an amount sufficient to assure completion of the <br />remaining reclamation work at the site if the work had to be performed by the Division, <br />including the cost of reestablishing vegetation on any revegetated areas, should those areas <br />fail. The Division estimated its cost to complete remaining reclamation work at the site to <br />be $22,562 (cost estimate dated April 23, 1999). The $22.562 estimate remains current; <br />therefore, the petmittee has adequate bond on deposit with the Division. <br />PART III -REQUESTED REVISIONS OF PERMIT APPLICATION <br />Based on its midterm review, the Division finds that permit application C-80-003 contains <br />the six deficiencies described below. As a result of these findings, the Division requests that <br />the permittee submit a Technical Revision to correct these deficiencies by October 31, 1999. <br />The changes to the permit application document may be incorporated into the permit <br />application as a supplemental volume. Correction of most of the deficiencies is necessary to <br />ensure compliance with Section 2.03.3(1) which requires that all information in the permit <br />application he current. (In following items, "new permit boundary" refers to the boundary <br />resulting from the recent Phase II/III partial bond release.) <br />1. New permit boundary not adequately delineated in permit application.
