Permit application, including stipulations and responses: <br />2. Revisions made to the permit since the last permit renewal; <br />3. Annual reclamation and hydrology reports; <br />4. Bond surety and reclamation cost estimate; <br />5. Division findings documents and previous midterm reviews; <br />6. Division inspection reports; <br />7. Notices of violations since last permit renewal, if any. <br />PART II -SUMMARY OF: OPERATIONS, VIOLATIONS, REVISIONS TO <br />PERMIT, STIPULATIONS, VARIANCES, AND BOND <br />Operations <br />Reclamation of Hayden Gulch Mine was completed in Fall of 1987. Permit 80-003's last <br />renewal was effective February 11, 1997. The Division's proposed approval of a Phase III <br />(final) bond release request for all but 6.8 acres of the original 1269-acre permit area on <br />June 23, 1999. This proposed bond release decision became Yinal 30 days later. <br />The remaining 6.8-acre permit area consists of three shrub enclosure parcels and a slump <br />area. A letter in the file for Minor Revision MR-13, dated May 13, 1998, from the <br />Division to the pertnittee's agent (TerraMatrix) explains the remaining bond liability <br />periods for the 6.8 acres.
