One copy of the following must be submitted within 30 days after each calendaz yeaz to the BLM Chief <br />Branch of Solid Minerals: <br />I. Lithologic and any other logs of strata penetrated and conditions encountered. <br />2. Ihil(hole location map showing exact hole locations, each hole being appropriately identified <br />by number of letter with the data being tied to such number of letter designation. <br />3. List of drillhole numbers and depth drilled. <br />B. All groundwater monitoring welts must be completed and cemented so as to isolate atl aquifer intervals <br />which show significant head differences or changes in water quality in order to prevent mixing of <br />unlike waters. <br />C. All wells must be completed and abandoned in accordance with applicable laws of the State Engineer's <br />Office. <br />D. Upon final abandonment of the wells, all water-bearing zones shall be sealed with a cement plug <br />beginning 20 feet below the zone and ending 20 feet above the zone, or the entire well can be sealed <br />with cement from the bottom to within three feet of the surface. <br />E. Dry holes or holes sealed in intervals shall be permanently plugged with an expanding plug placed ten <br />feet below ground level followed by seven feet of cement. <br />F. The top three feet of casing shall be removed and the azea filled with soil during final abandonment. <br />G. Well completion data and aquifer test results shall be included in quarterly hydrologic reports submitted <br />to the Division. <br />STATUS: Terminated Mazch 5, 1990. <br />10 <br />
