in part or in full by the Self-Insured Debtors and claimants may <br />request that the Division of Labor determine whether they are <br />entitled to benefits and, if so, the amount of such benefits. <br />e. While a claim remains open and/or unresolved, a <br />claimant may request a hearing on any issue, including, without <br />limitation, the amount and payment of temporary disability <br />benefits, the amount and payment of permanent disability <br />benefits, the need and payment for medical treatment, the payment <br />of disfigurement benefits, and provision of vocational <br />rehabilitation. - <br />9. In addition to the open and/or unresolved claims listed <br />on Exhibit 1, there are workers' compensation claims which were <br />closed after the applicable Self-Insured Debtor paid all ordered <br />-- or~agreed to benefits."= A claimant may request that a closed case <br />be reopened, within certain statutory time periods, in order to <br />seek additional benefits upon a worsening of his or her injury. <br />10. In addition to the known claims listed on Exhibit 1, <br />there may be claims, unknown to the Self-Insured Debtors, for <br />benefits related to alleged injuries prior to the Petition Date <br />which have not yet been asserted against the Self-Insured <br />Debtors. Subject to the limitations and requirements of the <br />Colorado Act and other applicable statutes and rules of procedure <br />and practice, such claims may be asserted against the <br />Self-Insured Debtors in the future. <br />li.
