<br />(h) Monitoring devices -inspections to ensure proper installation of <br />temperature probes, lysimeters, and other monitoring devices. <br />(3) Post -Construction inspections, including, but not necessarily limited to: <br />(a) Subgrade. <br />(b) Waste rock emplacement. <br />(c) Crushed limestone layer. <br />(d) Soil liner -inspections for cracks, holes, and other defects. <br />(e) Thermal protection drain layer. <br />(f) Topsoil -inspections for erosion damage, and to ensure adequacy <br />of sediment controls and vegetation. <br />(g) Drainage ditches -inspections for erosion damage. <br />(h) Monitoring devices -inspections to ensure proper functioning of <br />temperature probes, lysimeters, and other monitoring devices. <br />(D) The sampling and testing activities to be performed, including discussion of, and <br />frequency of, tests to be conducted on each component of the facility, such as the <br />subgrade, waste rock, crushed limestone layer, soil liner, thermal protection layer <br />and topsoil. <br />(E) Documentation regarding the construction of the facility, including daily summary <br />reports, inspection reports, acceptance reports, record drawings and final reports. <br />3. LAC Minerals shall have CQA samples sent to the laboratory, as soon as practicable, <br />following collection to optimize turnaround time. This will allow for timely action on <br />repairs necessitated by test results. Construction which would inhibit the ability to conduct <br />repairs shall not be initiated until laboratory results have been evaluated to either confirm <br />proper construction or determine if repairs are needed. When material is rejected because <br />observation and tests indicate that the material does not meet design specifications, further <br />testing shall be conducted to define the extent of the problem and to allow adequate <br />corrective measures to be implemented. The material shall be reworked, or replaced, and <br />retested until it meets the design requirements. Any component of the facility whose <br />construction is contingent on the completion of the failed component, shall not commence <br />until the failed component meets design requirements. <br />4. LAC Minerals shall employ qualified independent CQA consultants to perform inspections <br />and testing of ail of the components of the facility required to have CQA plans. LAC <br />Minerals shall submit the names of the CQA personnel prior to each phase of construction <br />of the waste impoundment. Upon Department request LAC Minerals shall submit the <br />resumes or a statement of qualifications for the CQA personnel. LAC Minerals shall <br />provide updated CQA personnel names and qualifications as necessary throughout the <br />project. <br />5. LAC Minerals shall have designated CQA personnel on the site at all times during the <br />construction of the clay liner of the waste impoundment.
