<br />following: <br />(1) Permitting agency; <br />(2) Facility owner/operator; <br />(3) Design engineer; <br />(4) CQA personnel; and <br />(5) Construction contractor or personnel. <br />(B) The qualifications, training and duties of the CQA personnel shall be described for <br />the following: <br />(1) CQA officers; <br />(2) CQA inspection personnel; and <br />(3) Consultants. <br />(C) The inspection activities to be performed, including discussion of all the <br />inspections and checks to be conducted on each component of the facility, include: <br />(1) Preconstruction inspections, including, but not necessarily limited to: <br />(a) Subgrade -review of site investigation information, and insections <br /> to identify any unexpected site conditions. <br />(b) Soil liner - materials testing for uniformity and to ensure <br /> specifications are met with respect to permeability, moisture <br /> density, and particle size. <br />(c) Thermal protection/drain layer - leachate testing to ensure water <br /> standards are met. <br />(d) Topsoil -nutrient testing for proper fertilizer application. <br />(2) Cons truction inspections, including, but not necessarily limited to: <br />(a) Subgrade - inspections to ensure design and compaction <br /> requirements aze met. <br />(b) Waste rock emplacement -inspections to ensure lift thickness and <br /> compaction requirements aze met. <br />(c) Crushed limestone layer - inspections to ensure thickness, <br /> uniformity, and compaction requirements are met. <br />(d) Soil liner -inspections to ensure removal of undesirable debris, <br /> thickness, uniformity, moisture, compaction, bonding between lifts, <br /> reworking, and permeability requirements aze met. Also <br /> inspections to ensure protection of the clay liner from desiccation, <br /> erosion, and freezing. <br />(e) Thermal protection/drain layer -inspections to ensure thickness <br /> requirements are met. <br />(f) Topsoil -inspections to ensure thickness and coverage requirements <br /> are met. <br />(g) Drainage ditches -inspections to ensure design specifications are <br /> met, including proper placement of geotextile, and proper sizing <br /> and placement of rip rap. <br />8 <br />
