2. FINAL DEPOSITION OF THE ON-SITE CONCRETE NEEDS TO BE CLARIFIED IN THE PERMIT. <br />SHOULD EFCI WISH TO PURSUE AN ALTERATION FROM THE PRESENTLY APPROVED POST-MINING <br />CONTOURS AND CONFIGURATION AT THE LOADOUT, THIS ISSUE NEEDS TO BE ADDRESSED IN A <br />TECHNICAL REVISION TO BE REVIEWED AND APPROVED BY THE DIVISION. <br />4. IN ORDER TO VERIFY FACILITY BACKFILL VOLUMES, THE OPERATOR WILL PROVIDE AN ACCURATE <br />CURRENT TOPOGRAPHIC/CONTOUR MAP WTCH APPLICABLE CROSS-SECTIONS TO REVISE MAPS C7 AND <br />7. <br />TILE ABOVE ISSUES ARE TO BE RESOLVED IN A REVISION(S) TO BE SUBMI'PI'ED WITHIN 6O DAYS OF <br />PERMIT RENE WAL. <br />Status: Continued, parts 1, 2, and 4 have been complied with. Part 3 remains outstanding. <br />Section V -Decision to Require Revisions <br />As a result of this midterm review, the Division will require revision(s) of the permit to address the <br />following concerns with the currently approved Southfield Mine permit application. <br />Rules 2.03.4 and 2.03.5 <br />With respect to rules 2.03.4 and 2.03.5, please update all Legal and Financial and Compliance <br />information and Ownership and Controls Irlformation as necessary. <br />Rule 2.03.6 <br />2. Since the Newlin Creek Mine has undergone a Phase III Bond Release, it is the belief of the <br />Division that EFCI no longer has legal right-of--entry to this area. Please provide right-of--entry <br />information for the topsoil stockpile located at the Newlin Creek Mine. <br />Rule 2.10.3 <br />With the use of Pond 1 as a disposal area for demolition debris and Pond 5 being used for both <br />ponds' subwatersheds, please update Map 20 -Drainage and Sediment Control (delineating each <br />of Pond 5's subwatersheds and eliminating Pond 1). Please provide three certified copies of this <br />map. <br />4. With the use of Pond 1 as a disposal area for demolition debris, please update Map 18 -Mine <br />Surface Facilities, to reflect that this pond no longer exists. Please provide three certified copies <br />of this map. <br />With the approval of Minor Revision 41, and the associated retention of the loadout truck tunnel, <br />the Division is assuming that the post-mining topography of the facilities azea will change. Please <br />provide a new post-mining topography map (map 28) and new post-mining cross-sections (map <br />7). Please provide three certified copies of each map. <br />
