-z- <br />I. Decision to Require Revision <br />The Division has conducted a inid-term review under Rules 2.08.3, 2.06.2 (a), <br />2.06.3(4), 2.06.5(3), 2.06.7(5) and 3.02.2(4). The Division finds that <br />certain revisions and modifications are needed to insure future compliance <br />with the Colorado Surface Coal Mining Reclamation Act and the Regulations of <br />the Colorado Mined Land Reclamation Board for Coal Mining. The following <br />section details the revisions required and a justification for requiring the <br />revisions. Section IA responses should be submitted as a request for a minor <br />revision by December 31, 1984. <br />IA. Revisions Required <br />Legal, Financial, Compliance Information - The Division finds that the <br />following information is inaccurate and needs to be updated to reflect <br />changes that have occurred over the past 2 1/2 years. Specifically: <br />a. A revised table of contents must be submitted for the permit <br />application. The table of contents must include all additional or <br />revised exhibits and maps, and a list of all supplemental material that <br />has been submitted, including stipulation responses. This information <br />is necessary to insure that the permit application is accurate and <br />complete. <br />b. The permit application form found in the front of Volume I must be <br />revised to show current names, addresses, contacts, etc. As an example <br />a new contact person must be provided in place of Mr. Thomas Hansen. <br />c. Current insurance certificates proving adequate coverage for <br />personal injury and property damage must be submitted to the Division. <br />d. The NPDES discharge permit in the application expired on September <br />30, 1984. Evidence of renewal must be submitted to the Division. <br />2. Subsidence Information <br />J An agreement between National King Coal, <br />refers to periodic inspections of an irrigation <br />workings of NKC's King Coal Mine.
