Lang <br />Divisbn Daecta <br />On April 30, 2001 the Division of Minerals and Geology found Western Mobile Northam, Inc. to have satis5ed <br />the appbcable requirements of C.RS.34-32.5-10] ~ ~. for obtaining a mining and reclamation pemut. <br />Therefore, a perilvt is being issued. Two signed originals of the permit have been executed. We have kept one <br />copy far our files and are enclosing one copy for your use. It is your responsibility to comply with all of the terms <br />of the pemlit. <br />1. All of the original application materials, as am®ded amd supplemwted, aze am integral part of your <br />permit. They have bear incorporated into fhe permit by reference. We presume that you have a copy of all of <br />these materials; therefore, none have been enclosed with this mailing. We suggest that you keep a copy of the <br />permit amd the pemut applicatioD at Ule mining operation as a reference for operating persomel, to help ensure <br />corr~liance with the terms oftlre permit. <br />2. Changes in the mining and reclamation operations that differ from those deSCdbed hl the permit may <br />require a modi5catioD to the permit. We suggest consuhang the Construction Materials Rules and Regulatioms <br />and/or coDtacting us to determine if a revision ro the pemut is necessary. Rule 1.10 pertains to Amendmems, <br />Rule l.9 to Teclmical Revisions, and Rule 1.1 I to Conversions. <br />3. On your permit awiversary date each year, April 30th, you must submit art annual fee and an amual <br />report to us. The annual fee for this pemtit is $688.00.
