3. Automobile Liability (Minimum). The contractor must carry a minimum of $600,000 combined <br />single limit. <br />VENDOR OFFSET <br />Pursuant to C.R.S. 24-30-202.4 (as amended), the State Controller may withhold debts owed to <br />state agencies under the vendor offset intercept system for: (a) unpaid child support debt or child <br />support arrearages; (b) unpaid balance of tax, accrued interest, or other charges specified in <br />Article 22, Title 39, C.R.S.; (c) unpaid loans due to the student loan division of the Department of <br />Higher Education; (d) owed amounts required to be paid to the unemployment compensation <br />fund; and (e) other unpaid debts owing to the State or any agency thereof, the amount of which <br />is found to be owing as a result of final agency determination or reduced to judgment as certified <br />by the controller. <br />PROJECT WORK DESCRIPTION <br />Item 1 -Equipment Rental <br />This item includes machine time using aturbo-charged excavator. Machine size will be a medium <br />sized, CAT 3206E or equivalent excavator with a minimum 20-foot reach and operator. Excavator <br />time is expected to perform initial excavation of collapsed mine features, recontouring and pull-back <br />for slope alignments with surrounding topography as well as site hydrologic control work, and other <br />machine work as directed. Equipment Rental will be in accordance to specifications (19.0) in the <br />General Bid Specifications, 1996. Equipment time will start as directed by the PROJECT <br />MANAGER from the location as marked on the site map. <br />Measurement and Payment <br />Payment for this task will be reflected under Item 1 of the Bid Schedule as an hourly rate for a CAT <br />3206E or equivalent excavator and operator. Bid hourly rate shall reflect the CONTRACTOR'S <br />estimate for equipment and personnel costs per hour to complete the task. Measurement will be <br />actual hours required to complete work as directed and approved by the PROJECT MANAGER <br />performed on-site as shown on the location map. <br />Item 2 - Rinra <br />Item 2 includes the procurement, transport and placement on-site of minimum 36-inch diameter <br />(least dimension) riprap. Riprap will be hard, durable, non-sedimentary in origin. Placement will be <br />defined as in the subsidence feature(s) after preparation is complete at the mine-site. The 30 tons <br />per bid schedule is NOT guaranteed. <br />Measurement and Payment <br />Payment for this task will be reflected under Item 2 of the Bid Schedule as per ton placed riprap. The <br />riprap cost shall reflect the CONTRACTORS estimate for procuring, transporting and placement of <br />riprap to complete the task. Measurement will be actual tons of riprap required, to complete work <br />as directed and approved by the PROJECT MANAGER. Weight tickets will be required in order to <br />be submitted for payment. <br />
