Harris Project Bid (PKA-0-1081) Dfvislnn Director <br />Enclosed is a brief description and bid schedule for a mine site in Routt county southeast of Hayden, <br />Colorado in the NW/4, Section 22, T6N, R87W on the Mt. Harris 7'/z minute USGS quadrangle. The <br />project consists of the initial excavation, importing of riprap, backfilling, recontouring, and <br />revegetation of a subsidence feature and the backfilling of an airvent related to abandoned mine <br />workings. <br />Site Showing <br />There will NOT be a pre-bid meeting, but contractors are encouraged to visit the site prior to <br />submitting a bid. <br />Bid Submission and Evaluations <br />Bids shalt be submitted by 4:00 p.m. on May 25, 2000 by fax, mail or in person to Yvonne Brannon. <br />Telephoned bids will not be accepted. The attached bid schedule must be submitted by all bidders. <br />Award will be made to the lowest grand total bid. <br />Bid Award <br />The apparent low bidder must submit a certificate of insurance with the following limits: <br />Standard Worker's Compensation and Employer Liability, as required by State Statute <br />including occupational disease, covering all employees at the work site. <br />2. General Liability (Public Liability & Property Damage) (Minimum); <br />a. Combined single limit of $600,000 written on an occurrence basis; <br />b. Any aggregate will not be less than $1 million; <br />c. The contractor must purchase additional insurance if claims reduce the annual <br />aggregate below $600,000; <br />d. The State of Colorado must be named as additional insured on each comprehensive <br />general liability policy; <br />e. Certificates of insurance must be provided to the State within 10 working days after <br />receipt of Award; and, <br />f. Insurance shall include provision providing notification to the State 10 days prior to <br />cancellation of the coverage. <br />
