The pipeline shall be buried to a minimum depth of 36 inches, except in areas where <br />bedmek is encountered, in which case the pipeline shall be buried to a minimum depth of <br />24 inches. At all times and situations, the holder is responsible to bury the pipe to a depth <br />wltich will safely accommodate all permirted uses of the mad and extreme eondiAons <br />related to weather, such as freezing and heavy rains. <br />10. Pipeline warning signs shall be installed within 60 days of construction completion. For <br />safety purposes, each sign shall be permanently marked with the right-of--way serial <br />number and shall clearly identify underground location, owner and purpose of the buried <br />pipeline. <br />1 J . Once the pipeline is constructed, the holder shall restore the existing roadway to meet or <br />exceed conditions prior to construction. The pre-construction driving width shall be <br />restored and erosion control structures installed subject to approval of the authorized <br />officer. The holder shall be responsible for maintaining roads in good condition from the <br />beginning to completion of the operation. This may include, but is aot limited to blading <br />the driving surface, cleaning ditches and drainage facilities, dust abatement, or other <br />requirements as directed by the authorized officer. <br />12. Trash shall be confined in a covered container while the project is in progress. Upon <br />completion, all trash, flagging, laths, etc. shall be removed and hauled to an authorized <br />disposal site. No burning of trash, trees, brush or any other material shall be allowed. <br />13. Under non-emergency conditions, BLM must be notified within 48 hours before mine <br />water is dischazged on public lands. If water is discharged due to an emergency, BLM <br />must be notified within 24 hours after release or observation of by-pass disclrazge. <br />] 4. Sampling and analysis of any discharged water will be in accordance with the Colorado <br />Department of Public Health and Environtnent (CDPI3E) National Pollution Dischazge <br />Elimination System (NPDES), also known as the Colorado Dischazge Permit System, <br />Copies of all samplnrg/analysis reports will be provided to BLM at the time when the <br />report is made available to OOSI or its agent. <br />I5. Monitorng will be performed on schedule to ensure that the mine drainage system <br />remains functional and in good repair. Inspections will include, but are not limited to: <br />manometer, vault, valves and dischazge points. Inspections will be performed monthly <br />for the first full season of operation, then quarterly thereafter to include, at the minimum, <br />the months of April and July. Unless otherwise authorized by BLM, the regular <br />monitoring period shall be considered April through December, but may be suspended <br />each year af[er flow had peaked and ceased. Monitoring is to resume in any given yeaz <br />shotild conditions occur that could reasonably result in a significant increase of discltarge. <br />Inspection repotts shall be documented and forwazded to BLM quarterly or as appropriate <br />to monitoring schedule. <br />2 <br />
