new problem.
Locatable Reference Point:
The map submitted in April 1998 does not have a known locatable point for reference.
Resnonse:
The Updated map submitted herewith has a reference tied to a nearby section corner.
Vent Sites and Access Roads not on Map:
The updated map was to include the ventilation shafts and access roads associated with
this Permit; these features were not shown on the map.
Response:
The new 1" = 200' and 1" = 100' maps submitted herewith show all features associated
with this Permit.
Map not in Conformance with Rule:
The map submitted by the Operator in April 1998 does not conform with the
requirements of Rule 6. Therefore, the Corrective Action is not corrected.
Response:
New maps submitted herewith meet the requirements of Rule 6, subject to Division
review. A Reclamation Map has not been submitted. This map cannot be provided until the
Division and Operator agree on how to resolve the outstanding issues of Affected Area boundary
and determination of which remote features will be included under this Permit. Upon reaching
concurrence with the Division on these matters, the Operator will submit the requisite
reclamation map.
December 19, 2001 Insaection by Bob Oswald:
No Permit Identification Sign or Permit Boundary Markers:
The lack of a Permit identification sign and Permit boundary markers is noted as a
"problem" and a Corrective Action was specified.
Response:
In January 2002 the Operator installed a correct Permit identification sign and provided
photographic evidence.
The Operator is submitting a Proposed Affected Area boundary herewith. Upon reaching
concurrence with the Division on the Affected Area boundary, the comers will be marked in the
field.
Inadequate Site Man:
Following the April 23, 1998 letter from the Division, which noted a number of items
that needed to be revised and updated on the site map, the Operator failed to provide a current
and accurate site map. As a result of the Operator's failure to respond to the Division's inquiries,
the lack of a current and accurate map is classified as a "problem" and a Corrective Action was
specified.
Response:
The Operator is submitting an updated and correct site map herewith. The map
accurately presents the specific items of concern noted by the Inspector. The Operator expresses
