<br />Mid-Term Review <br />November 27, 1985 <br />The appropriate surface facilities map(s) within the application need to be <br />revised to show the location and configuration of the heliport. This should <br />include Map 11, Surface Facilities Index 14ap. Drawing HP-O1 submitted with <br />the request should be incorporated into the application. Additionally, the <br />operations description portion of the application should be amended to include <br />the additional facility. <br />TR06 - Rail Loop Fill Slope Erosion Protection Plan - August 30, 1983 Request <br />Based on information contained in the Division files, it appears that the <br />final disposition of this plan was Division approval of a minor revision on <br />August 30, 1984 approving a sediment control system for the south facing rail <br />loop fill slope. The system consists of two ponds collecting disturbed area <br />runoff from the upper bench floors and a rock check dam-catch pond-haybale <br />system for the lower bench area (area east of the loadout pad). The <br />supporting hydrologic calculations for the approved design which were <br />submitted by Western Fuels on September 26, 1984 need to be incorporated into <br />the appropriate section of the permit application. Map 103A should be revised <br />or an additional map should be incorporated into the permit which shows the <br />location and configuration of the sediment control system for this area. This <br />map should be a certified as-built drawing of the installed system. <br />Additionally, the Operations Description portion of the permit should be <br />revised to include the added facility. <br />MR23 -Modify Revegetation Plan - September 8, 1983 Request <br />The revegetation plan portion of the permit application needs to be revised to <br />describe the delay in nitrogen fertilizer application until the second growing <br />season on topsoiled fill slopes and temporary laydown areas on the conveyor <br />access road and in the rail loop area. Cut slopes and non topsoiled fill <br />slopes should receive phosphorus fertilization prior to seeding. <br />TR07 - Stabilize Conveyor Road Cut Slopes -September 15, 1983 Request <br />A specific approval document for this technical revision could not be located <br />in the Division files. However, a commitment by the Division to approve the <br />plan is included in a February 8, 1984 letter from MLRD to the BLM. Please <br />check your files to see if a specific approval form was ever received by <br />Western Fuels. Permit application Maps 106 through 113 need to be revised to <br />delineate all cut slopes which will not be revegetated due to high rock <br />content. The cut slope along the east side of the conveyor road between Sta. <br />145 and 150 was not excluded from revegetation requirements as originally <br />proposed. <br />MR24 - Slot Storage Pad Sediment Control - November 22, 1983 Request <br /> <br />Map 105 needs to be revised and certified to show the as-built configuration <br />of the slot storage pad and associated drainage control system. <br />!' - <br />
