'- <br />~ ~ <br />Mid-Term Review 31 November 27, 1985 <br />Section V - Amendments to Permit Application <br />The following comments regard various portions of the permit application which <br />will need to be revised and/or updated to reflect the current status and <br />condition of the Deserado Mine. <br />1. Section I.E.I.b., items (1) through (4) of the application should be <br />reviewed and revised as necessary to show any changes incorporate officers, <br />partners, directors, principal shareholders, etc. <br />2. Section I.E.2 of the application must be amended to comply with the <br />requirements of Rule 2.03.5(3), items (a) through (f ). <br />3. Section I.E.5 of the application should be revised to reflect the current <br />status of the mine development schedule within the permit term. <br />4. Section I.E.6 - Personal Injury and Property Damage Insurance <br />Information. The Certificate of Insurance provided on Page I-25 of the <br />application has expired. This certificate needs to be replaced with anew <br />Certificate of Insurance which shows that the operation is presently covered <br />with a public liability insurance policy sufficient to meet the requirements <br />of Rule 2.03.9. <br />5. Hydrologic Ionitoring Information - Sections II.C., II.D.B and V.B. The <br />Division has prepared a table (attached Table I) which delineates the current <br />hydrologic monitoring program for the Deserado Mine. This table also includes <br />a listing of all current NPDES discharge points and a description of the <br />organization and format of the Annual Hydrology Report to be prepared by <br />- Western Fuels and submitted to the Division on January 1 of a given year. <br />6. Miscellaneous Revisions to Text: <br />Section II - Environmental Resources - This section of the application <br />should be revised to incorporate any additional baseline data or other <br />reports prepared subsequent to permit issuance. This information would <br />include such items as the Red Wash AVF Study and the Big Game and Sage <br />Grouse Lek Survey report submitted in response to stipulations attached to <br />the permit. <br />Section III - Mine Description - The description of mine entry development <br />should be revised to reflect modifications to the mining sequence, layout, <br />and reduction in number of RP panels in accordance with technical and <br />minor revision approvals (TR09, MR28) granted subsequent to permit <br />issuance. Figure III-3 should also be revised accordingly. <br />Figure III-5 should be amended to show the as-built sediment control <br />system for the ventilation facility. <br />Figure III-7 should be revised to show the correct refuse subarea <br />development sequence. <br />
