The violation was abated and the notice terminated on August <br />19, 1994, when the Division received inspection reports, from a registered professional engineer, for the <br />ponds and impoundments. <br />The operator was fined $750.00. <br />Notice of Violation C-94-036 <br />Issued December 1, 1994. <br />The nature of the violation was: <br />"Failure to design, construct, and maintain the Northwest Sediment Trap to provide for <br />either a combination of principal and emergency spillways, or a single open channel spillway <br />to safely pass a 25-year, 24-hour precipitation event." <br />The violation was abated by filling in the trap so that it no longer impounded water and would not be <br />subject to the requirements for impoundments The notice was terminated on July 7, 1995. <br />The operator was fined $350.00. <br />Meadows No. 1 Mine (C-81-029) February 6, 1996 <br />Midterm Review 4 Third Permit Tcrm <br />
