• - monitoring the placement process of fill to ensure that CONTRACTOR exercises <br />care in the vicinity of pipes and that the underlying geosynthetics are not <br />damaged; and <br />- CONTRACTOR verification of in-place Soil Liner Fill and Drain Cover Fill <br />thickness. <br />PART 6.0 DOCUMENTATION <br />An effective quality assurance program depends on thorough monitoring and documentation <br />of all construction activities. This is most effectively accomplished by observation and <br />documentation during all phases of construction. Documentation shall consist of daily record- <br />keeping, construction problem resolutions, design and SPECIFICATION changes, photographic <br />records, weekly progress reports, chain of custody forms for test sample tracking, and a <br />certification and summary report. <br />6.01 DAILY RECORD KEEPING <br />Daily records shall consist of field notes, observation and testing data sheets, summary of the <br />daily meeting with CONTRACTOR, and reporting of construction problems and resolutions. <br />This information shall be submitted on a regular basis to MANAGER for review. <br />6.02 SOILS OBSERVATION AND TESTING FORMS <br />Soils observation and testing forms generally include the following information: <br />- date, project name, location, and weather data; <br />- a reduced-scale site plan showing work areas and test locations; <br />- descriptions of ongoing construction; <br />- summary of test results and samples obtained, with locations and elevations; <br />- off-site materials received including quarry certificates; <br />- test equipment calibrations, if necessary; and <br />- signature or initials of the COA Monitor. <br />6.03 CONSTRUCTION PROBLEM AND SOLUTION SHEETS <br />Construction problem and solution sheets generally include the following information; <br />- detailed description of the problem; <br />- location and cause of the problem; <br />- how and when the situation or deficiency was identified; <br />how the problem was resolved; <br />- any measures taken to prevent similar problems in the future; and <br />February 18, 1994 Section 01400.1 <br />01400.1-8 Earthworks COA Plan <br />Revision 0 <br />
