- signature of the CQA Monitor and MANAGER. <br />6.04 PHOTO DOCUMENTATION <br />All phases of construction shall be photographed. Photographs shall be identified by location, <br />time, date, and name of the CQA Monitor taking the photograph. <br />6.05 DESIGN AND SPECIFICATION CHANGES <br />During construction, the need to address DESIGN and SPECIFICATION changes, modifications, <br />or clarifications may arise. In such cases the COA Monitor shall notify MANAGER, which <br />would in turn notify the Engineer. DESIGN and SPECIFICATION changes shall only be made <br />with written.agreement from MANAGER and Engineer. <br />6.06 WEEKLY PROGRESS REPORTS <br />The COA Monitor shall prepare weekly progress reports summarizing all construction and <br />quality assurance activities. This report shall be submitted to MANAGER and shall include the <br />following information: <br />- date, project name, and location; <br />- summary of construction related activities; <br />- summary of samples taken and test results; <br />- summary of deficiencies and/or defects and resolutions; and <br />- signature of the CQA Monitor. <br />6.07 CERTIFICATION REPORT <br />At the completion of the project, the COA Monitor shall submit to MANAGER a certification <br />report. This report shall certify that the work has been performed in compliance with the <br />DRAWINGS and SPECIFICATIONS and will contain the following information: <br />- summary of all construction activities; <br />observation and test data sheets; <br />- sampling, testing locations, and test results; <br />- a description of significant construction problems and the resolution of these <br />problems; <br />- changes to the DRAWINGS or SPECIFICATIONS and the justification for these <br />changes; <br />record drawings, and <br />- a certification of closure statement signed and sealed by an engineer registered <br />in the State of Colorado. <br /> <br />February 18, 1994 Section 01400.1 <br />01400.1-9 Earthworks COA Plan <br />Revision 0 <br />
