• 4.03 DEFICIENCY MEETINGS <br />Special meetings will be held, as needed, to discuss potential problems or deficiencies. At a <br />minimum, these meetings will be attended by the COA Monitor and CONTRACTOR. If the <br />problem relates to a design issue, MANAGER and Engineer should also be present. The <br />meeting will be documented by the COA Monitor. <br />When deficiencies litems that do not meet contract requirements) are discovered, the CQA <br />Monitor will immediately determine the nature and extent of the problem and notify <br />CONTRACTOR. If unsatisfactory test results identify a deficiency, additional tests will be <br />performed to define the eMent of the deficient area. <br />CONTRACTOR shall correct the deficiency to the satisfaction of the COA Monitor. If <br />CONTRACTOR is unable to correct the problem, the COA Monitor will notify MANAGER which <br />will assist in problem resolution. If the solution involves a design revision, the Engineer shall <br />also be contacted. <br />The corrected deficiency shall be retested and/or approved before any additional related work <br />is performed by CONTRACTOR. All retests and related documentation shall be recorded by the <br />COA Monitor and included in the final report documentation. <br />PART 5.0 EARTHWORKS CONSTRUCTION QUALITY ASSURANCE <br />Construction of the heap leach facility or specified earthworks shall be in accordance with the <br />DRAWINGS and SPECIFICATIONS. A COA monitoring and testing program shall be <br />implemented to ensure construction compliance. The CQA testing program shall consist of <br />. construction testing of materials used in the heap leach facility construction. The types of <br />materials are defined in the SPECIFICATIONS. During construction, these soil types shall be <br />sampled and tested to determine if they meet SPECIFICATIONS. Soil samples shall be obtained <br />in accordance with American Society for Testing and Material standards ASTM D75 and ASTM <br />D420. All tests shall be performed on-Site or in a geotechnical laboratory approved by <br />MANAGER. <br />5.01 CONSTRUCTION TESTING <br />Durinrconstruction, all earthwork components shall be tested to verify that the construction <br />is in accordance with the SPECIFICATIONS. Testing shall be performed on all in-place soils <br />used in the construction to confirm tfie materials meet SPECIFICATIONS. The tests to be <br />performed, including testing frequency, for each material type are listed in Tables 1 and 2. The <br />testing frequencies specified in Tables 1 and 2 shall be increased when construction conditions <br />ladverse weather, equipment breakdown, improperly ballasted compactor, excessive lift <br />thickness, improper soil type, degree of moisture conditioning and compaction is douhtfull <br />warrant additional tests. Additional tests will be approved by MANAGER and directed by the <br />COA Monitor. <br />5.02 CONSTRUCTION MONITORING <br />All earthwork components of the construction shall be monitored and tested to verify that the <br />construction is in accordance with the SPECIFICATIONS. The COA Monitor shall identify <br />inadequate construction methodologies or materials that may adversely impact the performance <br />of the facility heing constructed and existing structures.
