The final location of ttre pipeline represents a field change from the original design location
(as authorized by Mr. Steve Shuey on June 17, 1999) due to wetter than anticipated
conditions in the vicinity of the proposed location that resulted in equipment operating
limitations.
All areas of the NW tsiiling pond recontoured surface were covered with an approximate 6-
inch lift of colluvial growth medium. Outslope areas of the crest berm were originally
constructed with colluvium and exhibit significant volunteer vegetation which was not
disturbed during the reclamation activity.
The borrow area located west of the mine entrance road was minimally utilized (area of
disturbance approxima3tely 20-feet by 20-feet) as a borrow source and remains substantially
intact in terms of recontoured slope and volunteer vegetation. On conclusion of next year's
(Phase II) activities, this area will be dressed off in entirety. Camp Bird Colorado, Inc.
estimates a 2°I° withholding is sufficient to cover final reclamation of the borrow area.
Refer to photographs 2-A and 2-B in Appendix A.
Division Calculated Bond Amount: $9,711.00
Bond Release Request Amount (98%): 39,517.00
tem I-7: Excavate drainage diversion for routing surface flows off of the tailings
ponds with minimal erosion.
The NE tailing pond was recontoured to facilitate positive drainage with erosion mitigation
measures incorporated; therefore, no diversion other than that already in place (as the
County Road 361 shoulder ditch) is required to address run-on/run-off.
The NW tailing pond was configured with the internal drainage (as described above in Item
A-2) and associated HDPE pipeline. Any upslope sourced run-on is diverted by the County
Road 361 shoulder ditch. Refer to photographs 3-A and 3-B in Appendix A.
Division Calculated Bond Amount: $848.00
Bond Release Request Amount (100%): 3848.00
Item I-8: Place average 9" diameter rip-rap in diversion.
Per Item I-7, drainage vas addressed via installation of the 18-inch diameter HDPE pipeline.
As such, no rip-rap is required as associated with the "diversion". Refer to photographs 3-A
and 3-B in Appendix A.
Division Calculated Bond Amount: $3,675.00
Bond Release Request Amount (100%): x3,875.00
Page 3 of 6
