<br /> <br />The applicant will need to submit the notarized agreement or engineering evaluation for <br />Steamboat Valley Road where it is located within 200 feet of the affected area. <br />Rule 6.4.6 Exhibit F- Reclamation Plan Map The applicant did not provide an adequate <br />reclamation plan map noting the expected physical appearance the area of the affected land. <br />A map will need to be submitted depicting the proposed topography of the area including <br />contour lines and sufficient detail to portray the direction and rate of slope of all reclaimed <br />lands. <br />6. Rule 6.4.12 Exhibit L- Reclamation Costs The Division will recalculate the amount of bond <br />required to reclaim the additional disturbance under this conversion application. The <br />applicant will be notified of the increase prior to the Division's decision due date. <br />Rule 6.5(4) Geotechnical Stability Exhibit The Division received the Vibra-Tech Engineers <br />report for blasting at the Masonville Quarries. The applicant proposes to follow the <br />suggestions in the report for the Sprague-Red Lyons Quarry. Please inform the Division the <br />distance of the closest structure (i.e. house, well, garage, etc...) to the blasting area. The <br />maps located in the file do nut show surrounding structures that may be affected by the <br />blasting operations. <br />The Division is under the assumption that the previously approved blasting plan under M-81- <br />057 will be followed. A copy of the plan is enclosed. Please inform the Division if there are <br />any changes to this plan. <br />Please be advised that the Sprague- Red Lyons conversion application may be deemed <br />inadequate, and the application may be denied on June 15, 1999 unless the above mentioned <br />adequacy review items are addressed to the satisfaction of the Division. If you feel more time is <br />needed to complete your reply, the Division can grant an extension to the decision date. This <br />will be done upon receipt of a written waiver of your right to a decision by July 15, 1999 and <br />request for additional time. This must be received no later than the deadline date.
