In addition, field personnel prepared <br />daily observation reports. Golder submitted these daily reports and a daily summary report to CC&V. <br />Daily reports are not included in this report but were available for Colorado Division of Minerals and <br />Geology (DMG) review during the course of the project. <br />1.3.2 Weekly Construction Summary Reports <br />Golder prepared weekly construction summary reports, which were submitted to CC&V and to the <br />DMG. Weekly reports for the period of September 2001 through August 2002 are presented in the <br />October 2002 Phase Na Certification Report, weekly reports from the period of September 2002 <br />through June 2003 are presented in the June 2003 Phase Nb PSSA Certification Report, and weekly <br />reports during the period of June 2003 through October 2003 are presented in the October 2003 <br />Phase Nb OSA Certification Report. The weekly reports during the period of April 2004 through <br />October 2004 aze presented in Appendix D. <br />1.4 Project Specifications <br />• <br />A11 work was performed in accordance with the design plans and specifications in Amendment No. 8 <br />to Mining Permit M-1980-244 and subsequent revisions as approved by DMG. Issued for <br />Construction (IFC) drawings are presented in the Drawings section of this report and project <br />specifications aze presented in Appendix E. No specification changes were documented during the <br />Phase Nc 2004 construction season. <br />1.5 Record Survey Documentation <br />Ames Construction of Aurora, Colorado provided the Record Drawings for the activities associated <br />with the Phase Nc OSA. A Colorado Registered Land Surveyor prepared the drawings. The Record <br />Drawings are presented in Appendix A.
