• certifications prior to geosynthetics deployment. Golder performed geosynthetics CQA activities in
accordance with the CQA Plan. Geosynthetics CQA activities performed included:
• Observation and documentation of geomembrane deployment, trial seams, seaming,
destructive testing, and non-destructive testing (pressure tests and vacuum tests);
• Performing conformance testing of geomembrane;
• Measuring geomembrane panel thickness and confirming adequate panel overlap;
• Visually inspecting geomembrane sheet quality, sheet grinding, seam alignment,
and continuity;
• Retaining archive samples of geomembrane destructive tests and trial seams; and
• Observation and documentation of geogrid deployment for remediation of
underground workings.
1.3 Project Coordination
Golder CQC and CQA activities were directed by Golder. The Golder Resident Engineer for the
period September 2001 to December 2002 was Mr. Bob Nichols, and the Golder Project Resident for
(. the remainder of Phase Nc construction was Mr. Ron DiDonato. The Certifying Engineer,
Mr. John F. Lupo, Ph.D., P.E. or his representatives (Ms. Kimberly Finke Morrison, P.E., R.G.,
Senior Project Engineer, and Mr. James M. Johnson, P.E., Principal) made periodic site visits to
resolve technical issues and to review Golder activities. Golder staffing from the period of September
2001 through August 2002 are presented in the October 2002 Phase Na Certification Report, staffing
from the period of September 2002 through June 2003 are presented in the June 2003 Phase Nb
PSSA Certification Report, and staffmg from the period of June 2003 through October 2003 are
presented in the October 2003 Phase Nb OSA Certification Report. Appendix C presents a summary
of the Golder staff on-site during the period of April 2004 through October 2004.
Representatives of CC&V, Ames, Poly-Flex, and Golder conducted daily, weekly, and
preconstruction coordination meetings throughout the construction period.
