Phase IVc Summary No. 12 Page 2 043-2170
• Ames continued production of bentonite-amended SLF at the screen plant and pug mill located above
the Ames materials yazd. Ames also continued production of Drain Cover Fill (DCF) material at the
screen plant by the secondary crusher.
Poly-Flex continued deployment of 80-mil single-sided textured LLDPE geomembrane in the Phase
Nc OSA. Geomembrane was installed in the Phase IVc OSA on the south facing slope of Raven Hill
downward to the Phase NbJNc tie-in. Poly-Flex deployed 80-mil single-sided textured LLDPE
geomembrane over an azea of approximately 128,317 square feet in the Phase Nc OSA during this
reporting period. Geomembrane panels C-434 through C-484 were deployed, seamed, and tested by
Poly-Flex. Destrucfive fusion and extrusion seam samples DFc-158 through DFc-173 and DXc-16
through DXc-17 were mazked and removed. Destructive fusion and extrusion seam peel and shear
testing on samples DFc-152 through DFc-166 and DXc-16 were performed and passed per project
specification.
Ames continued hauling, placement and grading of DCF material into the Phase Nc OSA in the
western and central portion of the VLF, Raven Hill area. Ames placed DCF material and High Volume
Solution Collection (HVSC) piping per the project plans onto approved geomembrane panels.
r 1
L_J
I •
A map of the azea worked during this reporting period is provided in Attachment C.
Field Monitorin¢ and Testing Activities
Golder observed construction activities associated with Phase Nc earthworks. All construction
materials and soil materials placed were verified as suitable, and construction methods were in
accordance with project specifications.
Golder observed excavation of the LDT outside the Phase IVc for trench dimensions and slope.
Golder observed placement, compaction, and moisture conditioning of SLF in the Phase Nc OSA for
subgrade prepazation, thickness, compaction, moisture-content, and materials used. The Golder CQA
earthworks monitor performed fifteen nucleaz moisture-density tests (SLNc-127 through SLNc-141).
Field test results aze presented in Attachment B.
Golder observed geomembrane deployment, documenting ambient temperature, panel dimensions,
thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure testing). Geomembrane
installed in the Phase IVc OSA met the project specification.
Golder visually observed the placement of DCF in the Phase IVc OSA azea. Golder CQA monitors
(one monitor per dozer when dozers aze greater than 50 feet apart) observed DCF placement and
verified minimum thickness of 2 feet for finish grade, and 4 feet for haul traffic (i.e., Caterpillaz
Mode1740 articulated trucks).
Golder observed placement of HVSC piping in the Phase Nc OSA, verifying installation and layout
per the project plans.
en~rvo ~zrwy zs-Imyli zooa.doc GOIder ASSOCiateS lnC.
