Phase IVc Sununary No. 11 Page 2 043-2170 <br />deployment. <br />• Ames continued limited production of bentonite-amended SLF at the screen plant and pug mill located <br />above the Ames materials yazd. Ames also continued production of Drain Cover Fill (DCF) material at <br />the screen plant by the secondary crusher. <br />Poly-Flex continued deployment of 80-mil single-sided textured LLDPE geomembrane in the Phase <br />IVc OSA. Geomembrane was installed in the Phase FVc OSA on the south facing slope of Raven Hill <br />downward to the Phase IVb/IVc fie-in. Poly-Flex deployed 80-mil single-sided textured LLDPE <br />geomembrane over an area of approximately 47,184 square feet in the Phase IVc OSA during this <br />reporting period. Geomembrane panels C-426 through C-433 were deployed, seamed, and tested by <br />Poly-Flex. Destructive fusion seam samples DFc-152 through DFc-157 were mazked and removed. <br />Destructive fusion seam peel and sheaz tests of samples DFc-121, DFc-122, DFc-126, and DFc-129 <br />were performed and passed per project specification. <br />Ames continued placement and grading of DCF material into the Phase IVc OSA in the western and <br />central portion of the VLF, Raven Hill azea. Ames placed DCF material and High Volume Solution <br />Collection (HVSC) piping per the project plans onto approved geomembrane panels. <br />A map of the azea worked during this reporting period is provided in Attachment C. <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase IVc earthworks. All construction <br />• materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder observed excavation of the Phase IVc anchor trench for proper location, dimension (2' wide by <br />3' deep) and maintenance prior to geomembrane deployment. Anchor trench excavation met or <br />exceeded the requirements of the specification. <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVc OSA for <br />subgrade prepazation, thickness, compaction, moisture-content, and materials used. The Golder CQA <br />earthworks monitor performed ten nucleaz moisture-density tests (SLNc-117 through SLNc-126) and <br />performed one nucleaz moisture-density retest on SLF that required reworking (SLNc-124a) located <br />within the Phase Nc OSA. The retested SLF met the requirements of the specifications. Field test <br />results are presented in Attachment B. <br />Golder observed geomembrane deployment, documenting ambient temperature, panel dimensions, <br />thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure testing). Geomembrane <br />installed in the Phase IVc OSA met the project specification. <br />Golder visually observed the placement of DCF in the Phase IVc OSA azea. Golder CQA monitors <br />(one monitor per dozer when dozers aze greater than 50 feet apart) observed DCF placement and <br />verified minimum thickness of 2 feet for finish grade, and 4 feet for haul traffic (i.e., Caterpillar <br />.
