Phase IVc Summary No. 5 Page 2 043-2170 <br />• Ames hauled, placed, and compacted Soil Liner Fill (SLF) material into the Phase Nc OSA in the <br />central portion of the VLF. SLF material was tested, conditioned, reworked, retested, and maintained <br />prior to SLF surface acceptance. <br />Ames continued production of bentonite-amended SLF at the screen and pug mill plant located above <br />the Ames materials yazd. Ames also continued production of Drain Cover Fill (DCF) material at the <br />screen plant located neaz the secondary crusher. <br />Poly-Flex continued deployment of 80-mil single-sided textured LLDPE geomembrane in the Phase <br />IVc OSA. Geomembrane was placed in the Phase IVc OSA on the east-facing slope of Chickenhawk <br />Hill and onto the Saddle Fill area, at the Phase IVb/IVc tie-in. Poly-Flex deployed 80-mil single-sided <br />textured LLDPE geomembrane over an area of approximately 375,634 square feet in the Phase Nc <br />OSA during this reporting period. Geomembrane panels C-118 through C-236 were deployed, seamed, <br />and tested by Poly-Flex. Destructive fusion and extrusion seam samples DFc-037 through DFc-082 <br />were mazked and removed. Destructive fusion and extrusion seam peel and sheaz tests on 80-mil <br />single-sided textured LLDPE geomembrane samples DFc-037 through DFc-060 were tested during this <br />reporting period and passed per project specifications. <br />Ames continued placement and grading of DCF material into the Phase IVc OSA, on the west area of <br />the VLF on Chickenhawk hill. Ames placed DCF material onto approved geomembrane panels and <br />high volume solution collection piping per the project plans. <br />• A map of the azea worked during this reporting period is provided in Attachment C. <br />Field MonitorinE and Testine Activities <br />Golder observed construction activities associated with Phase IVc earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder observed construction of LDT in Phase IVc for trench dimensions, slope, and materials used. <br />Golder observed excavation of SU in Phase IVc for trench dimensions and slope. <br />Golder observed anchor trench excavation and backfill in the Phase IVc OSA for proper location and <br />dimension (2' wide by 3' deep). Anchor trench excavation met or exceeded the requirements of the <br />specifications. <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVc OSA <br />for subgrade preparation, thickness, compaction, moisture content, and materials. The Golder CQA <br />Earthworks monitor performed fifteen nuclear moisture-density tests (SLNc-054 through SLNc-69) <br />and performed three nucleaz moisture-density retests on SLF that required reworking (SLNc-57a, <br />63a, and 67a) located within the Phase IVc OSA.
