Phase IVc Summary No. 5 Page 3 043-2170 <br />• Golder observed geomembrane deployment, documenting ambient air temperature, panel <br />dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, and <br />destructive). Geomembrane placed in the Phase IVc OSA met the project specifications. <br />Golder visually observed the placement of DCF in the Phase IVc OSA azea. Golder CQA monitors <br />(one monitor per dozer when dozers aze greater than 50 ft apart) observed DCF placement and <br />verified minimum thickness of 2 feet for finish grade, and 4 feet for haul traffic. <br />Golder observed placement of secondary and tertiary high volume solution collection piping in the <br />Phase IVc OSA, verifying construction and layout per the project plans. <br />Golder visually observed removal of DCF June 11 from the Phase IVc OSA area, to allow for <br />inspection of the Phase IVc geomembrane and HVSC piping due to equipment damage. Golder <br />mazked the defect damage (19S) caused to the geomembrane for subsequent repair and non- <br />destructive testing. <br />Laboratory Tests <br />Golder obtained a QC sample of bentonite-amended soil liner fill (SLF-QC-12). Laboratory index <br />testing was conducted on samples of bentonite-amended SLF at either the Golder Soils Laboratory in <br />Lakewood, Colorado, or at the field trailer to verify suitability of materials prior to use in the field. A <br />summary of QC laboratory test results for SLF materials aze available at the Golder field trailer. <br />• Golder obtained QA samples of soil liner fill (SLFc-QA-OS, and SLFc-QA-06) and drain cover fill <br />(DCFc-O1) for laboratory testing. Testing of sample SLFc-QA-06 is not yet complete. The samples <br />tested met the requirements of the specifications and results aze presented in Attachment B. <br />Meetines <br />CC&V held a Weekly Contractors' meeting on June 8, 2004. Items discussed included Safety, <br />Environmental, Schedule, QC documentation, and CQA.
