Phase IVc Summary No. 4 Page 3 043-2170 <br />Golder observed placement, compaction, and moisture conditioning of SLF in the Phase IVc OSA <br />• for subgrade preparation, thickness, compaction, moisture content, and materials. The Golder CQA <br />Earthworks monitor performed thirty-one nucleaz moisture-density tests (SLNc-023 through SLNa <br />53) and performed seven nucleaz moisture-density retests on SLF that required reworking (SLNc- <br />23a, 23b, 25a, 27a, 30a, 34a and 42a) located within the Phase IVc OSA. The retested SLF met the <br />requirements of the specifications. Field test results aze presented in Attachment B. <br />Golder observed geomembrane deployment, documenting ambient air temperature, panel <br />dimensions, thickness, seaming, tie-in overlap, and seam testing (trial seams, pressure, and <br />destructive). Geomembrane placed in the Phase IVc OSA met the project specifications. <br />Golder visually observed the placement of DCF in the Phase IVc OSA azea. Golder CQA monitors <br />(one monitor per dozer when dozers aze greater than 50 ft apart) observed DCF placement and <br />verified minimum thickness of 2 feet for finish grade, and 4 feet for haul traffic. <br />Laboratory Tests <br />Golder obtained QC samples of bentonite-amended soil liner fill (SLF-QC-10, and SLF-QC-11) and <br />drain cover fill (DCF-QC-O1). Laboratory index testing was conducted on samples of bentonite- <br />amended SLF and DCF at either the Golder Soils Laboratory in Lakewood, Colorado, or at the field <br />trailer to verify suitability of materials prior to use in the field. A summary of QC laboratory test <br />results for SLF materials are available at the Golder field trailer. <br />• Golder obtained QA samples of soil liner fill (SLFc-QA-02, SLFc-QA-03, and SLFc-QA-04) for <br />laboratory testing. SLFc-QA-04 is incomplete and will be reported in the following weekly. The <br />completed samples tested met the requirements of the specifications and results aze presented in <br />Attachment B. <br />Meetincs <br />CC&V held Weekly Contractors' meetings on May 25 and June 1, 2004. Items discussed included <br />Safety, Environmental, Schedule, QC documentation, and CQA.
