Phase IVc Summary No. 4 Page 2 043-2170 <br />Ames continued excavation of the Phase IVc OSA geomembrane anchor trench in the western portion <br />• of Phase IVc on Chickenhawk hill, excavating approximately 1000 lineaz feet of anchor trench in <br />preparation for geomembrane deployment. <br />Ames hauled, placed, and compacted Soil Liner Fill (SLF) material into the Phase IVc OSA, on the <br />west area of the VLF. SLF material was tested, conditioned, reworked, retested, and maintained prior to <br />SLF surface acceptance. <br />Ames continued production of bentonite-amended SLF at the screen and pug mill plant located above <br />the Ames materials yazd. Ames began production of Drain Cover Fill (DCF) material at the screen <br />plant by the secondary crusher. <br />Poly-Flex continued deployment of 80-mil single-sided textured LLDPE geomembrane in the Phase <br />IVc OSA. Geomembrane was placed in the Phase fVc OSA on the east-facing slope of Chickenhawk <br />Hill and onto the Saddle Fill azea, at the Phase IVb/IVc tie-in. Poly-Flex deployed 80-mil single-sided <br />textured LLDPE geomembrane over an area of approximately 284,923 squaze feet in the Phase IVc <br />OSA during this reporting period. Geomembrane panels C-37 through C-117 were deployed, seamed, <br />and tested by Poly-Flex. Destructive fusion and extrusion seam samples DFc-005 through DFc-036 and <br />DFx-001 through DFx-005 were marked and removed. Destructive fusion and extrusion seam peel and <br />sheaz tests on 80-mil single-sided textured LLDPE geomembrane deployed during this reporting period <br />passed per project specifications. <br />Ames began placement and grading of DCF material into the Phase IVc OSA, on the west area of the <br />• VLF. Ames placed DCF material onto approved geomembrane panels and high volume solution <br />collection piping per the project plans. <br />A map of the azea worked during this reporting period is provided in Attachment C. <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase IVc earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. <br />Golder observed placement and compaction of SF in the Phase IVc OSA for subgrade prepazation, <br />lift thickness, compaction, moisture content, and materials used. The Golder CQA Earthworks <br />Monitor performed visual observations to verify conformance with method specifications. SF <br />placed met the requirements of the project specifications. <br />Golder observed construction of LDT in Phase IVc for trench dimensions, slope, and materials used. <br />Golder observed anchor trench excavation in the Phase IVc OSA for proper location and dimension <br />(2' wide by 3' deep).
