Note the location of any significant, valuable, and <br />permanent man-made structures within 200 feet of the affected <br />area such as :houses, barns, fences, above or below ground <br />utilities, irrigation ditches, maintained or public roads, and <br />communication antennas. A narrative description should be <br />provided in Exhibit B - Site Description; and, <br />6. Outline and label existing disturbance within and/or <br />adjacent to thc; permit boundary (e.g., previously mined area, <br />roads or excavs~tions resulting from utility construction) . If <br />you redisturb areas that have been previously disturbed, then <br />they must be .included in the permit area and addressed in <br />Exhibit D - Reclamation Plan. <br />Reclamation Plan Map <br />1.
