<br />repre:>entative samples of the injection/production <br />fluids; <br />(b) a convenient and acceptable attachment point at <br />each wellhead on the tubing for a standard 1/2 <br />inch male threaded pressure gauge (the final as- <br />built design must be approved by the EPA, prior to <br />insta.llation); <br />(c) for ain injection well, a pressure gauge attached <br />to th~a convenient and acceptable attachment point <br />of each tubing to allow for injection pressure <br />monitoring. The gauge shall be designed to <br />operate at a certified deviation accuracy of <br />approximately five percent (5$), throughout the <br />range of anticipated injection pressures; <br />(d) a flow meter with cumulative volume recorder that <br />is certified for at least ninety-five percent <br />(95$) accuracy, throughout the range of injection <br />rates allowed by the permit; <br />(e) a continuous recorder set up to monitor the <br />tubing/long string casing annulus pressure and <br />designed to operate at a certified deviation <br />accuracy of approximately five percent (5$); <br />(f) digital temperature logging equipment, located at <br />the wellhead, designed to monitor the temperature <br />of either the injected or produced fluids; <br />(g) an instrument designed to measure the depth to the <br />air/fluid interface within the tubing/casing <br />annulus, for wells of both the Pilot Project and <br />commercial phase; and <br />(h) a continuous recording instrument designed to <br />adequately monitor the gel in the long <br />string/wellbore annulus. <br />During the Pilot Project, the permittee has the opportunity <br />to demonstrate that both visual monitoring of gel levels at land <br />surface and temperature surveys (two methods proposed by the <br />permittee), adequately measure the effectiveness of the gel. <br />5. Proposed Changes and Workovers. The permittee shall <br />give advance notice to the Director, as soon as possible, of any <br />planned physical alterations or additions to any permitted well. <br />Major alterations or workovers of any permitted well shall meet <br />all conditions as set forth in this permit. A major <br />alteration/workover shall be considered any work performed, in <br />which the tubing string is completely removed or the casing is <br />Page 7 of 61 <br />EPA Permit No. COS2358-0696 <br />
