.~ • <br />PART I:I. SPECIFIC PERMIT CONDITIONS <br />A. WELL CONSTRUCTION REQUIREMENTS <br />1. Casing and Cementing for Pilot Project. The <br />construction details submitted with the application are hereby <br />incorporated into this permit as Appendix A-1, and shall be <br />binding on the permittee. For the Pilot Project, one (1) pair of <br />wells may be cased a:nd cemented in the manner proposed by the <br />permittee. The company may request that two (2) additional well <br />pairs, on an individual case by case basis, be incoporated into <br />the Pilot Project if said well pairs are necessary for further <br />testing. <br />A Cement Bond Log (CBL) shall be run in conjunction with the <br />following logs: gamima ray; casing collar locator; amplitude <br />curve or pipe energy curve; transit travel time curve; and full <br />wave display. The CBL shall be submitted to the EPA prior to <br />commencement of operation. <br />2. Casing and Cementing for Commercial Project. The <br />casing program submitted with the application is hereby <br />incorporated into this permit and shall be binding on the <br />permittee. The permittee is required to use sufficient <br />quantities of cement to secure the 8-5/8 inch casing (long <br />string) such that this casing is cemented from the casing shoe to <br />the top of the Mahogany Zone (Appendix A-2). <br />A Cement Bond Loq (CBL) shall be run in conjunction with the <br />following logs: gamma ray; casing collar locator; amplitude <br />curve or pipe energy curve; transit travel time curve; and full <br />wave display. Any CBL shall be submitted to the EPA prior to <br />commencement of operation. <br />3. Tubing and Packer Specifications. The permittee <br />proposes to utilize a packerless completion, with the intent of <br />monitoring the annular/injection fluid interface. The annular <br />space between the tubing and long string casing will contain air, <br />compressed to approximately 967 pounds per square inch gauge <br />(psig). <br />During normal operations, the air/injection fluid interface <br />must be maintained at a depth of no more than 100 feet above the <br />long string casing shoe. Injection between the outermost casing <br />protecting underground sources of drinking water (USDWs) and the <br />wellbore is prohibited. <br />4. Monitoring Devices. The operator shall provide and <br />maintain in good operating condition, for each injection or <br />production well: <br />(a) a tap on the line, for the purpose of obtaining <br />Page 6 of 61 <br />EPA Permit No. COS235B-0696 <br />
