<br />affected. <br />Demonstration of mechanical integrity shall resume <br />immediately, followi.nq completion of workovers/alterations, in <br />accordance with Part II, Section C. 4. The permittee shall <br />provide all records of well workovers, logging, or other test <br />data to EPA within thirty (30) days of completion of the <br />activity. Appendix B contains samples of the appropriate <br />reporting forms. <br />6. Lopping Specifications. As discussed in the sections <br />on casing/cementing :requirements, the permittee is required to <br />run a suite of logs 'to accompany each CBL, to assess the quality <br />of cement bonding between the long string casing and the <br />wellbore. <br />The permittee shall run a cement bond log (CBL)-variable <br />density log (VDL)-gamma ray log (GR) from the base of the 8-5/8 <br />casing to a depth of 100 feet above the predicted top of the <br />primary cement. Along with this suite of logs, or any other <br />pertinent logs, the permittee shall include appropriate narrative <br />interpretations by a knowledgeable log analyst. <br />7. Postponement of Construction. If no injection and/or <br />production well is constructed within two (2) years of the <br />effective date of this permit, the permit will automatically <br />expire; unless the permittee requests an extension. The request <br />shall be made to the Director in writing, in lieu of the <br />quarterly reporting requirements of Part II, Section D. 5., and <br />shall state the reasons for the delay. The extension under this <br />section may not exce~sd two (2) years. <br />Financial responsibility shall be maintained during the <br />period of inactivity in accordance with Part II, Section F. Once <br />a permit expires under this part, the full permitting process, <br />including opportunity for public comment, must be repeated before <br />authorization to injfact will be re-issued. <br />B. CORRECTIVE ACTION <br />The following wells, within the area of review, may <br />potentially be affected by the solution-mining operations and are <br />not currently serving as monitoring wells: IRI-PW-1, IRI-PW-2, <br />Skyline-1, Minerals :'.8-1, Dunn 20-1, and Cross V A1. <br />The permittee shall set a cement plug across the Boies Bed <br />and Dissolution Surface for each of the above-referenced wells. <br />Each plug shall be preceded by sufficient amounts of mud-acid <br />wash and fresh water, so as to insure a good cement bond to the <br />wellbore and/or casing, if applicable. <br />Page 8 of 61 <br />EPA Permit No. CO52358-0696 <br />
