~- a <br />u <br />43.31(h) <br />(h) The Licensee shall notify the Agency within 24 hours of the following: <br />(1) any spill that extends: <br />(i) beyond the wellfield monitor well ring; <br />(ii) more than 400 feet from an injection or production well <br />pipe artery to or from a recovery plant; or <br />(iii) more than 200 feet from a recovery plant; or <br />(2) any spill which exceeds 2,000 gallons and which exceeds the <br />concentrations for water listed in Appendix 21-A, Table II, <br />Column 2, in Part 21 of these rules. <br />n <br />U <br />43.32 Expiration and Termination of License <br />(a) Except as provided in 43.33(b) and 43.32(d)(4), each specific license <br />shall expire at the end of the day, in the month and year stated in <br />the license. <br />(b) Each licensee shall notify the Agency immediately, in writing, and <br />request termination of the license when the licensee decides to <br />terminate all activities involving materials authorized under the <br />license. This notification and request for termination of the license <br />must include the reports and information specified in 43.32(d)(1)(iii) <br />and (f). The licensee is subject to the provisions of 43.32(d) and <br />(e), as applicable. <br />(c) No less than 90 days before the expiration date specified in a <br />specific license, the licensee shall either: <br />(1) submit an application for license renewal under 43.33; or <br />(2) notify the Agency in writing, under 43.32(b), if the licensee <br />decides to discontinue all activities involving radioactive <br />material. <br />(d) (1) If a licensee does not submit an application for license renewal <br />under 43.33, the licensee shall on or before the expiration date <br />specified in the license: <br />(i) terminate use of radioactive material; <br />(ii) properly dispose of radioactive material; and <br /> <br />(iii) submit a record of disposal of <br />radiation survey(s) of license <br />use and/or storage. Levels of <br />as required by 21.108, 21.109, <br />measurement instrument(s) used <br />shall be specified. <br />43 - 9 <br />radioactive material and <br />a's permanent location(s) of <br />radiation shall be reported <br />and 43.90(1).
