<br />LJ <br />~~ <br />OTHER PERMITS AND LICENSES REQUIRED EXHIBIT M <br />The following permits aze needed for the site: <br />1) County Special Use - A full Special Use Permit is needed with the County Planning and <br />County Commissioner's office. This permit has been approved for the current operation but will <br />need to be amended to include the additional acreage being added to the existing permit. The <br />operator will be submitting a new application for this permit concurrently with the DRMS <br />amendment application. <br />2) NPDES combined process water/storm water dischazge permit - A NPDES pemut will be <br />needed from the Colorado Department of Public Health and Environment since water will be <br />pumped offsite to the Gunnison River. This permit has been obtained. <br />3) APEN - A fugitive air emissions permit is needed from the Colorado Department of Public <br />Health and Environment since the site will mine more than 70,000 tons per yeaz. Air emissions <br />permits will be in place for all equipment utilized on site which requires a permit. The APEN <br />has been obtained. <br />4) Gravel Well Permit - A Gravel Well Permit with the Colorado Division of Water Resources <br />must be obtained since the site will partially be reclaimed as a lake. This permit includes an <br />augmentation plan and substitute water supply plan for the evaporation from the lake surface <br />created. These permits have been applied for and aze in the review process. <br />5) A CDOT Road Access Permit will be required for vehicles entering 1800 Road. This permit <br />has been submitted and approved by CDOT. A permit from CDOT and UPRR to install <br />acceleration and deceleration lanes on SH 92 will also be needed. This permit has been <br />submitted to and approved by CDOT; approval from UPRR is pending. United Companies is <br />also working with UPRR on the installation of electronic gates at the RR crossing for 1800 Road. <br />Anderson Pit, Apri107 M-1 <br />
